PurposeBased on the theory of behavioral goal model,this study explored the core competence of specialist nurses in the direction of organ transplantation nursing,and preliminarily constructed the training system of specialist nurses in line with China’s national conditions,so as to provide theoretical reference for the training of specialist nurses in the direction of organ transplantation nursing in China.Methods:1.Status survey: To select the specialist nurses in the field of organ transplantation nursing in hospitals with organ transplantation qualifications in Shandong Province as the research object,the study investigated the influencing factors of the core competence of specialist nurses in the field of organ transplantation nursing by using the "core competence scale of specialist nurses in organ transplantation",providing the basis for the construction of the later training index system.2..Literature review: The research group used keywords such as "organ transplantation","specialist nurse","core competence","index system"" as search terms to search domestic and foreign literature databases,Medical College websites and Nursing Society websites.The entry pool of the specialist nurse training index system for organ transplantation nursing direction based on core competence was initially formed.3.Semi-structured interview: A semi-structured interview was conducted with clinical nursing experts and specialized nurses in the direction of organ transplantation nursing.Colaizzi’s seven step analysis method was used to integrate and analyze the interview data,extract the main topics related to the influencing factors of the core competence of specialized nurses in the direction of organ transplantation nursing,and further supplement the item pool of the index system.4.Delphi expert consultation: The selected correspondence experts conducted two rounds of correspondence on the first draft of the index system item pool,and finally determine the expert nurse training index system for organ transplantation nursing based on core competence.Results1.Current status survey results: A total of 219 valid questionnaires were collected through the status quo survey.The average score of core competence of specialist nurses in organ transplantation nursing was(216.84 ± 39.58),which was at the upper middle level.The degree of love for nursing career,whether they have received professional training in organ transplantation,technical titles,initial education and nursing work years were statistically significant(P < 0.05),which were the main influencing factors of the core competence of specialist nurses in organ transplantation nursing direction.2.Results of literature review: 121 effective literatures were finally included.According to the degree of correlation,the literature was divided into two groups: The first group of indirectly related literatures(48 articles),mainly including organ transplantation,nurse core competence,specialized nurse training,related policy documents,etc.,aims to understand the new trends of organ transplantation and the current research status of nurse core competence,and lay the foundation for the construction of this study.The other group was directly related literature(73 articles),which mainly included core competence and influencing factors of organ transplantation nurses,training mode of core competence of nurses,curriculum and clinical practice,and a pool of 55 index system entries was initially formed.3.Semi-structured interview results: After combing and analyzing the interview content,three themes were finally summarized,which were the training mode of specialist nurses in organ transplantation nursing based on core competence,the curriculum and clinical practice arrangement of specialist nurses in organ transplantation nursing based on core competence,and the assessment and evaluation of the training quality of specialist nurses in organ transplantation nursing based on core competence.According to the literature review,the survey results and the semi-structured interview results,the preliminary index system of specialist nurse training in organ transplantation nursing direction includes 4 first-class indicators,18 second-class indicators and 122 third-class indicators.4.Results of Delphi expert consultation: Fifteen experts from five provinces completed two rounds of questionnaires.The positive coefficients of experts in the two rounds were 88.880% and 93.750%,respectively.The average authority coefficient of experts was 0.903,and the Kendall harmony coefficient was 0.343(P < 0.001)and 0.393(P < 0.001),respectively.The expert opinions tended to be consistent.Finally,an index system for specialist nurse training in organ transplantation nursing based on core competence was established,including 3 first-class indicators,16 second-class indicators and 128 third-class indicators.ConclusionsAfter two rounds of expert correspondence,this study initially constructed an index system for the training of specialist nurses in the direction of organ transplantation nursing based on core competence.The index system is scientific and can provide a reference for the training of specialist nurses in the direction of organ transplantation nursing in China.
