Objective:Oral lichen planus(OLP)is a chronic inflammatory disease of oral mucosa,which is mainly believed to be related to T cell-mediated immune disorders.Recent studies have found that glycolytic metabolism plays an important role in T cell proliferation,differentiation and secretion of inflammatory factors.The glycolytic metabolic rate is mainly influenced by Hexokinase 2(HK2),Phosphofructokinase-1(PFK-1)and Pyruvate kinase M2(PKM2)rate-limiting enzymes.Therefore,by detecting the expression of glycolytic rate-limiting enzyme in tissues and peripheral blood of OLP patients and its correlation with HIF-1α,this study explored the role of glycolytic metabolism in the occurrence and development of OLP,providing new ideas for the treatment of OLP.Methods:In this study,40 OLP patients admitted to the mucosal Department of Stomatology Hospital of Shanxi Medical University from May 2021 to December 2022 were selected in strict accordance with the inclusion and exclusion criteria,including 20 erosive and 20non-erosive patients,and 20 healthy people were selected as the control group.OLP tissues and normal oral mucosal tissues were collected;all tissue samples were confirmed by pathology physicians;Immunohistochemistry(IHC)was adopted to detect the expressions of glycolytic rate-limiting enzymes HK2,PFK-1,PKM2 and HIF-1α in the tissues of the three groups of subjects.Meanwhile,5ml of peripheral blood of all subjects were collected and the expression levels of glycolytic rate-limiting enzymes HK2,PFK-1,PKM2 and glycolytic metabolism regulator HIF-1α in plasma were detected by Enzyme-linked immunosorbent assay(Elisa).The correlation between the expression levels of HK2,PFK-1 and PKM2 in plasma and HIF-1α was analyzed.All the results were analyzed using SPSS26.0 software.Results:1.Glycolytic rate-limiting enzyme(HK2,PFK-1,PKM2)and glycolytic metabolism regulator HIF-1α were almost not expressed in normal tissues,but significantly increased in OLP tissues,and the expression was more obvious in erosive than non-erosive OLP tissues,the difference was statistically significant(P< 0.05).2.Compared with healthy subjects,the expression levels of glycolytic rate-limiting enzyme HK2,PFK-1,PKM2 and regulatory factor HIF-1α in plasma of OLP patients were increased,and the expression levels in plasma of erosive OLP patients were significantly higher than those of non-erosive OLP patients,and the difference between the three groups was statistically significant(P< 0.05).3.The expression levels of glycolytic rate-limiting enzymes HK2,PFK-1 and PKM2 in plasma of OLP patients were positively correlated with HIF-1α,and the correlation was HK2(rs= 0.672),PFK-1(rs=0.745),PKM2(rs= 0.744).Conclusion:Compared with healthy subjects,the expressions of glycolytic rate-limiting enzymes HK2,PFK-1,PKM2 and regulatory factor HIF-1α in tissues and peripheral blood of OLP patients were significantly increased,and the expression in erosive OLP group was significantly higher than that in non-erosive OLP group,suggesting that glycolytic rate-limiting enzymes may be related to the onset and severity of OLP lesions.The expression levels of HK2,PFK-1 and PKM2 in peripheral blood of OLP patients were positively correlated with HIF-1α,suggesting that HIF-1α may be related to the expression of glycolytic rate-limiting enzyme in OLP patients.
