| ObjectiveBased on clinical practice,this paper aims to analyze the difference of SIRT1 and IL-6 levels between patients with coronary heart disease and non-coronary heart disease,and to explore the correlation between SIRT1 and IL-6 levels and the severity of coronary artery stenosis,and to further explore whether they both service more evidence for coronary heart disease.MethodsAmong the patients hospitalized in the First Affiliated Hospital of Henan University from April 1,2021 to September 1,2021,136 patients diagnosed with coronary heart disease were selected,and they were divided into acute myocardial infarction group(n = 49),stable angina pectoris group(n = 38)and unstable angina pectoris group(n = 49).In addition,37 patients hospitalized were selected as the control group.Clinical data of all patients were collected in the case system of our hospital,and blood indicators were collected.Severity of coronary artery stenosis using the Gensini score method,using ELISA method to detect the level of SIRT1,IL-6.Finally,SPSS 25.0 software was used for statistical analysis to analyze the differences of SIRT1,IL-6 levels,between the observation group and the control group,and then to analyze the levels of SIRT1,IL-6 and Gensini scores among different groups of coronary heart disease.With Gensini score Spearman correlation analysis and correlation between SIRT1,IL-6,at the same time using Logistic regression analysis of independent impact factors of coronary disease.Along with,using ROC curve analysis SIRT1,IL-6 separate and combined to predict clinical diagnostic value of coronary disease.Results(1)This study included 205 patients,excluding 16 cases of previous myocardial infarction(7.80%),5cases of complicated with stroke(2.44%),severe infection(0.98%),3 cases of severe heart failure(1.46%),4 cases of gastrointestinal bleeding(1.95%),and autoimmune 2 cases(0.98%).Finally,173 patients meeting the inclusion and exclusion criteria,including 136 patients(78.61%)in the observation group and 37 patients(21.39%)in the control group.(2)Comparison of general clinical baseline dataThe average age of 173 patients in the study was 63(53,70)years old,100 males(57.80%)and 73females(42.20%).The mean age of patients in the observation group was 64(57,71),higher than 56.00(45.5,66.5)(P=0.002)in the control group.History of diabetes control group was 97.3%,the proportion more than 65.4% in the observation group(P < 0.001).Control group the Gensini score was 4.00(1.50,5.00)lower than 34.50(18.38,56.00)of the observation group(P < 0.001).Between the two groups of FPG,Hb A1 c,AST,ALT,Mb,c Tn T,CK-MB has a statistically significant,but other indicators were no differences.(3)Inter-group comparison of observation indicatorsObservation group of IL-6 levels of 10.08(7.30,12.54)pg/ml,higher than the control group7.65(6.34,8.94)pg/ml(P < 0.001).Observation group SIRT1 level of 0.91(0.69,1.27)ng/ml,lower than the control group 1.29(1.06,1.78)ng/ml(P<0.001).The IL-6 level of stable angina group was6.80(5.76,8.25)pg/ml,unstable angina group was 10.89(8.71,12.93)pg/ml,and acute myocardial infarction group was 11.77(9.67,13.04)pg/ml.Stable angina pectoris group is lower than the other two groups.The SIRT1 level of stable angina group was 1.21(0.85,1.48)ng/ml,unstable angina group was1.06(0.89,1.34)ng/ml,and acute myocardial infarction group was 0.76(0.51,0.88)ng/ml.The acute myocardial infarction group was lower than the other two groups.(P<0.05).(4)Correlation analysis of SIRT1,IL-6 level and Gensini scoreCorrelation analysis results show that the Gensini score are positively related with IL-6(rs=0.299,P<0.001),negatively correlated with SIRT1(rs=-0.321,P<0.001),positively related to age(rs=0.024,P=0.017)and glycosylated hemoglobin(rs=0.220,P=0.015),There was a positive correlation with alanine aminotransferase(rs=0.399,P=0.010),and a positive correlation with alanine aminotransferase(rs=0.233,P<0.001),and IL-6 was negatively correlated with SIRT1(r=-0.193,P=0.011).(5)Multivariate binary logistic regression analysis of coronary heart diseaseWhether coronary heart disease as dependent variable,age,fasting glucose,aspertate aminotransferase,alanine aminotransferase,SIRT1,IL-6 and glycated hemoglobin as independent variables for regression analysis,the results suggest SIRT1 is the independent protection factors in patients with CHD,Other indicators were independent risk factors for CHD.(6)Evaluation of the clinical diagnostic value of SIRT1 and IL-6 in predicting coronary heart disease alone and in combination by ROC curveThe AUC of independent prediction of IL-6 was 0.734(95% CI: 0.656-0.811,P<0.001).When the cutoff value was 9.504pg/ml,it was the most diagnostic value,with sensitivity of 55.9% and specificity of89.2%.The AUC of SIRT1 independent prediction was 0.727(95% CI: 0.639-0.816,P<0.001).When the cutoff value was 1.088ng/ml,it was the most valuable diagnostic value,with sensitivity of 75.7% and specificity of 64%.The AUC of combined detection was 0.834,and the sensitivity and specificity were72.8% and 89.2%,respectively(P<0.001).Conclusion1.SIRT1,IL-6 diagnosis in patients with coronary heart disease have a certain value,and the joint can obviously increase the diagnostic value. |
