Clinical Observation Of Zigui Tiaogan Decoction In The Treatment Of Decreased Ovarian Reserve Function Of Kidney Deficiency And Liver Depression

Objective:To evaluate the efficacy and safety of Zigui Tiaogan Decoction in the treatment of patients with diminished ovarian reserve with kidney deficiency and liver depression,and to provide new ideas for clinical treatment of diminished ovarian reserve.Methods:Using the method of self-control study,30 patients with DOR who met the diagnostic criteria were selected from April 2022 to November 2022 in the outpatient department of traditional Chinese medicine gynecology of Dongguan Maternal and Child Health Hospital,aged 18-39 years old.All the enrolled patients were treated with oral Zigui Tiaogan decoction.On the 5th day of each menstrual cycle,they began to take drugs until the next menstrual cramps,one dose per day,250 ml water decoction,once in the morning and once in the evening,for 3 consecutive menstrual cycles.According to follicle-stimulating hormone(FSH),luteinizing hormone(LH),FSH / LH,estradiol(E2),anti-mullerian hormone(AMH),antral follicle count(AFC)and TCM syndrome scores,the therapeutic effect was evaluated,and the efficacy and safety of Zigui Tiaogan Decoction were comprehensively evaluated.Results:28 cases were successfully completed.1.common data:The average age of 28 patients with decreased ovarian reserve function of kidney deficiency and liver depression was(33.93±3.8)years old,and the average course of disease was(17.07±6.55)months;2.Serum hormone levels:FSH and FSH/LH levels decreased after treatment,and the difference was statistically significant(P<0.05).After treatment,LH level decreased and E2 level increased,but the difference was not statistically significant(P>0.05);3.AMH:After treatment,the AMH value increased,and the difference was statistically significant(P<0.05);4.AFC:AFC increased after treatment,and the difference was statistically significant(P<0.05);5.In terms of TCM syndrome score:after treatment,the menstrual cycle,menstrual volume,backache,breast distending pain,less abdominal distending pain,chest discomfort,bitter mouth and dry throat,good sigh,depression or emotional irritability,dizziness,tinnitus and loss of libido were significantly improved(P<0.05),and the total effective rate of TCM syndrome score was 92.85%(26/28);6.the total effective rate was 82.1%(23/28).Conclusion:Zigui Tiaogan Decoction has a significant clinical effect in the treatment of patients with decreased ovarian reserve function of kidney deficiency and liver depression.It can effectively reduce the levels of FSH and FSH/LH,increase the level of AMH,and increase AFC.It has obvious curative effect in improving menstrual cycle disorder,less menstrual flow,soreness of waist,breast distending pain,less abdominal distending pain,chest and hypochondrium discomfort,bitter mouth,dry throat or good sigh,mental depression or irritability,dizziness,tinnitus and decreased libido.It has no obvious adverse reactions and high safety.It is worthy of clinical promotion.
