Objective:Through the collection of clinical characteristics and imaging data of patients with anterior communicating aneurysm in our hospital,statistical analysis was conducted on patients’ age,gender,morphological characteristics of aneurysms,hemodynamic parameters and other factors to explore the risk factors related to anterior communicating aneurysm rupture.Methods:Eighty-three patients with anterior communicating aneurysm admitted to the first affiliated hospital of gannan medical college from November 2019 to January 2022 were retrospectively included.According to the symptoms,signs or imaging findings of corresponding subarachnoid hemorrhage,the 83 patients were divided into unruptured group and ruptured group.Clinical characteristics(including age,gender,hypertension,diabetes,smoking,etc.),morphology of anterior communicating aneurysm(size,aspect ratio,size ratio,hypoplasia of anterior cerebral artery segment A1,Angle between anterior cerebral artery segment A1 and A2),and hemodynamics(including wall shear stress and concussive shear index)of the two groups were compared.And the multi-factor Logistics regression analysis of the risk factors.Results:There were statistically significant differences in age,history of cerebral infarction,aneurysm maximum diameter,aspect ratio,size ratio and standardized time average wall shear stress between the two groups(P < 0.05),while there were no statistically significant differences in gender,history of hypertension,history of diabetes,history of smoking,Angle between A1 dysplasia and A1 and A2 on one side,and oscillatory shear index between the two groups(P > 0.05).Multivariate binary Logistic regression analysis was performed for age,history of cerebral infarction,size,aspect ratio,size ratio,and standardized time average wall shear stress.Aspect ratio was retained as a risk factor for anterior communicating aneurysm rupture in the regression model(P=0.032,OR 11.131).A history of cerebral infarction was a protective factor(P=0.005 OR 0.154).Conclusion:Morphological characteristics such as cerebral infarction history,size,aspect ratio,and size ratio are related to anterior communicating aneurysm rupture.The aspect ratio >0.475 May be one of the risk factors for rupture.
