Objective:The purpose of this study was to analysis the clinical and morphological features of aneurysm factors involved determining treatment modalities for anterior communicating artery (AcoA) aneurysms.Methods:The authors retrospectively analysis of the second hospital of Shandong University in June2004-December2011meet the criteria for the92cases of patients with anterior communicating artery aneurysms before. Multiple variable including10variables were analysized by means of the univariable and multivariable Logistic regression analysis model SPSS18.0.The following factors were evaluated:the age and gender of patient,associated with medical disease, GCS grade, Hunt-Hess grade, the size of aneurysm, the neck size of aneurysm, dome-to-neck ratio, multiple lobulation and the dominance of A1segment of anterior cerebral artery.Results:56cases (60.8%) underwent surgical clipping,36cases (39.2%) were treated by intervention treatment.88(95.7%) were better prognosis,4cases (4,.3%) with poor prognosis. The statistic result showed surgical clipping, interventional treatment have no significant. The univariable analysis results showed age,aneurysm size.aneurysm neck size,multiple lobulation and the dominance of Al segment of anterior cerebral artery would bring statistical significance for choice of treatment method..The univariable and multivariable Logistic regression analysis indicate that the elderly patients (≥60years) are more suitable for intervention embolization. Conclusion:Age is the most important element to decide the treatment method. And aneurysm size, aneurysm neck size, multiple lobulation and the dominance of Al segment of anterior cerebral artery are independent factors to influence the choice of treatment method. Treatment method for anterior communicating aneurysm should not be decided by a single clinical factor or morphological feature, but should be decided after a comprehensive consideration various factors, and treatment for every patient should be personalized.
