| Objectives:By exploring and practicing the pharmaceutical service model of a compact medical alliance in the region,the SERVQUAL model was used to construct the quality evaluation system of pharmaceutical care in the medical community and evaluate the quality of pharmaceutical care,In order to test the practicality and applicability of the existing model.The evaluation aims to clarify the future development direction of pharmaceutical care in the city’s medical community,optimize the quality of pharmaceutical care in the medical community,promote the significant improvement of the level of pharmaceutical care in primary medical institutions,and provide reference experience for more medical regions to carry out the model construction and quality improvement evaluation of pharmaceutical care in the medical community.Methods:1.Model construction of pharmaceutical services in medical alliances: On the basis of fully participating in the construction,the current situation of the construction of pharmaceutical care in the medical alliance was grasps.Relevant experts and staff were invited to form an activity group together,and the main causes of the problems in pharmaceutical care in the early stage of the construction of the medical alliance were found by brainstorming method.The fishbone diagram analysis method was used to summarize the main causes,and the real causes were screened and summarized by the real cause inspection method.Combined with the existing influencing factors,practical work should be carried out step by step to explore and form a pharmaceutical care model of a medical alliance.2.Construction of the pharmaceutical service quality evaluation system in the medical community: An activity group was set up around the five dimensions of SERVQUAL model,and the medical workers were taken as the research objects.The factors affecting the satisfaction of pharmaceutical services in the medical community were brainstorming,and the fishbone diagram analysis method was used to effectively summarize the factors.A pre-questionnaire was designed,and the reliability and validity of the index were tested.To form a satisfaction evaluation scale for pharmaceutical care in a medical community and determine the index weight.3.Evaluation of pharmaceutical service quality in the medical community: After constructing a satisfaction model,a questionnaire was designed according to the evaluation indicators,and a survey was carried out in the targeted population,and the expected value and actual perception of each dimension index were statistically analyzed.Results:1.Model construction of pharmaceutical service in a medical alliance: Ten main factors affecting the construction of pharmaceutical service model in a city were identified,and concrete construction measures were taken based on the main factors and practices,thus forming a unified drug catalog,unified procurement,unified distribution,unified rules and regulations management and unified index assessment;The “five integration and three extension” pharmaceutical service model of SPD logistics service extension,prescription review extension and precision pharmaceutical service extension achieved the good effect of “one increase and two reductions” in pharmaceutical service management.The qualified rate of prescriptions in township hospitals increased from57.11% to 91.33%,and the average rate of antibacterial drug utilization in outpatient clinics decreased from 48.67% to 17.22%.The average rate of glucocorticoid use decreased from 8.11% to 4.52%.2.Construction of the pharmaceutical service quality evaluation system in the medical community: The service quality evaluation system was formed,covering five dimensions and a total of 21 service quality evaluation indicators.The weights of the five dimensions were 0.332,0.157,0.237,0.139 and 0.134,respectively,among which the maximum weight of the specific evaluation indicators was 0.496 and the minimum was 0.094.3.Pharmaceutical service quality evaluation of the medical alliance: An online questionnaire survey was conducted among the medical staff of municipal member units,township hospitals and village clinics involved in the construction of the medical alliance.A total of 234 valid questionnaires were received,with an effective questionnaire rate of 97.5%.Statistical analysis found that the expected value of each dimension index was higher than the actual perceived value,among which the highest value was the expected expectation score of tangibility(4.20),followed by empathy(4.19),and the expected expectation score of responsiveness and assurance(4.11 and4.12)was lower.Combined with the actual perceived average score,it can be seen that the service quality of each dimension in the construction of pharmaceutical service mode in the medical alliance fails to meet the overall expectations of relevant workers.Conclusions:The innovative exploration of the “five unified,three extended” pharmaceutical care mode of the medical community provides an excellent reference case for the construction of pharmaceutical care of the local medical community;the evaluation index system of pharmaceutical care quality in the medical community was constructed,which provided a scientific evaluation index reference for the quality evaluation of pharmaceutical care in various regions;through the evaluation,the problems in the current pharmaceutical care model of a city’s medical community were found,and the solutions were put forward,which effectively guided the improvement of the quality of the pharmaceutical care model of a city’s medical community. |
