ObjectiveThe symptom distress of elderly patients with type 2 diabetes comorbidity was investigated,the symptom groups and the influencing factors of each symptom group were analyzed,and the relationship between each symptom group and diabetic pain,self-efficacy,anxiety and depression was explored.MethodThis study is a cross-sectional status survey.Elderly patients with type 2 diabetes,admitted to a hospital in Dali from March to December 2022,and 396 elderly patients with type 2 diabetes comorbidities were selected in strict accordance with the inclusion and exclusion criteria,and the general demographic data questionnaire,symptom distress scale,diabetes pain scale,chronic disease self-efficacy energy scale and anxiety and depression scale were used for questionnaire survey.Descriptive statistical analysis was used to describe percentages,means,and standard deviations,and the differences of patients were compared by ANOVA,and Spearman correlation and multiple stepwise linear regression explored the relevant factors of patients’ symptom groups.Result1.The effective recovery rate of this study was 96.35%.Of the 411 questionnaires distributed,396 were valid,and 9 of the symptoms were > 40% of the elderly patients with type 2 diabetes comorbidities,including difficulty concentrating,pain,fatigue,dry mouth,numbness of hands and feet,lack of sleep,dizziness,irritability,and weight loss.2.After exploratory factor analysis,there were four symptom groups in this study,including somatic related symptom group,fatigue-related symptom group,autonomic nerve-related symptom group,and emotion-related symptom group;The incidence rates of the four symptom groups were 1.35± 1.86,0.66± 0.82,0.71± 0.82,0.86 ± 0.98,respectively.3.One-way ANOVA showed that there were significant scores of age,education level,ethnicity,marital status,economic resources,duration of diabetes,family history of diabetes,diabetes complications,number of types of chronic diseases and severity of symptom groups(P<0.05).4.Multiple linear regression analysis showed that the influencing factors of somatic-related symptom groups were ethnicity,marital status and economic resources;The influencing factors of fatigue-related symptoms were marital status,financial resources and awareness of diabetes;The influencing factors of the group of emotionally related symptoms were marital status;Autonomic nerve-related symptom clusters,there are no statistically significant influencing factor in this study.5.Correlation analysis showed that somatic-related symptom groups were negatively correlated with diabetic pain and positively correlated with self-efficacy;The fatigue-related symptom cluster was negatively correlated with diabetic pain and positively correlated with anxiety and depression in anxiety.Autonomic nerve-related symptom groups were positively correlated with diabetic pain and anxiety and depression.There were no significant significance between the emotion-related symptom clusters and the scales.ConclusionElderly patients with type 2 diabetes comorbidities experience complex symptoms,symptoms are clustered into four dimensions,which are affected by ethnicity,marital status,economic resources,whether there are complications,and the degree of awareness of diabetes,and at the same time,the four dimensions are related to diabetes pain,chronic disease self-efficacy and anxiety and depression,medical staff can strengthen assessment and care for the characteristics of elderly patients with type 2diabetes comorbidities,alleviate the symptoms of type 2 diabetes comorbidities,and improve their quality of life. |
