Objective:To investigate the cognitive impairment incomorbidity of type 2 diabetes and MDD,and to determine the relationship between changes in caspase-1 and IL-1?expression and cognitive impairment.To further explore the role of caspase-1 and IL-1? in the cognitive impairment incomorbidity of type 2 diabetes and MDD.Methods:A total of 162 people were enrolled,there were 49 patients in the comorbidity of type 2 diabetes and MDD group,54 patients in the MDD group and 45 patients in the type 2 diabetes group.Repeatable Battery for the Assessment ofNeuropsychological Status(RBANS)was used to assess the cognitive function of these three groups.Serum levels of caspase-1,IL-1?,apoptosis-associated speck-like protein containing a carboxy-terminal(ASC)and Nod-like receptor family pyrin domain containing 3(NLRP3)were determined by ELISA,the mRNA level of NLRP3 inflammasome were determined by real-time quantitative PCR detecting system(Q-PCR).Caspase-1,IL-1?,ASC,NLRP3 inflammasome and the mRNA expression as well as cognitive function were compared in the three groups.Results:1.Comparison of RBANS(including immediate memory,attention,and delayed memory):1)immediate memory:the immediate memory of the comorbidity of type 2 diabetes and MDD group(65.3 7 ± 16.39),which was lower than that of the MDD group(78.31±19.55)and the type 2 diabetes group(79.80±18.60),and the difference was statistically significant(P<0.05).2)attention:the attention of the comorbidity of type 2 diabetes and MDD group(85.20±16.80)was lower than that of the MDD group(92.15±16.06)andthe type 2 diabetes group(92.16±14.07),and the difference was statistically significant(P<0.05).3)delayed memory:the delayed memory ofthe comorbidity of type 2 diabetes and MDD group(75.76±17.00)was lower than that ofthe MDD group(86.30±14.36)and the type 2 diabetes group(92.16±14.43),and the difference was statistically significant(P<0.05).2.The comparison between the three groups of the level of caspase 1,IL-1?,ASC protein levels,NLRP3 inflammasome protein and the mRNA expression of NLRP3 inflammasome:1)Comparison of the caspase-1:thecaspase-1 of the comorbidity of type 2 diabetes(0.049(0.018?0.089))is higher than the caspase 1 of the MDD group(0.023(-0.052?0.060)),the difference was statistically significant(P<0.05).2)Comparison of the IL-1 ?:the IL-1? of the comorbidity of type 2 diabetes(0.244(0.193?0.300))were significantly higher than the IL-1? of theMDD group(0.229(0.128?0.300))and the IL-1? of the type 2 diabetes group(0.120(0.055?0.197)).The difference was statistically significant(P<0.05).3)Comparison of the ASC,NLRP3 inflammasome protein and the mRNA expression of NLRP3 inflammasome:There was no statistically significant difference among the three groups.3.Correlation study:1)Spearman correlation analysis:Caspase-1 protein was positively correlated withIL-1?,ASC and NLRP3 inflammasome(r=0.427,r=0.557,r=0.486,P<0.005).IL-1?protein was positively correlated withcaspase-1(r=0.427,r=0.564,r=0.596,P<0.005).2)Multivariate linear regression analysis:Caspase-1 was used as the dependent variable for stepwise linear regression analysis,and the results showed that caspase-1 was correlated with IL-1?,ASC,and immediate memory(F=80.160,p<0.001,adjusted R2=0.713).The equation was established as follows:y=0.005+0.457×ASC+0.271×IL-1?-0.001 × immediate memory.TakingIL-1? as the dependent variable,the stepwise linear regression showed thatIL-1? was correlated with ASC,caspase-1,NLRP3 and HAMD,F=97.793,p<0.001,and adjusted R2=0.800.The equation was established as follows:y=0.011+0.539×ASC+0.404×caspase-1+0.450×NLRP3+0.003×HAMD.Conclusion:The cognitive impairment of patients of the comorbidity of type 2 diabetes and MDD group was worse thanthe MDD group and the type 2 diabetes group,and the immediate memory,attention and delayed memory in the comorbidity of type 2 diabetes and MDD group were lower than those.in the other two groups.The differences of the caspase-1 protein and IL-1? protein betweenthe comorbidity of type 2 diabetes and MDD group,the MDD group and the type 2 diabetes group were significant,the level of the caspase-1 protein and IL-1? protein is higher than the other two groups,the cognitive impairment of three groups is associated with caspase-1 protein and IL-1? protein,suggesting caspase-1 protein and IL-1? protein may be involved in the cognitive impairment aggravation mechanism of the the comorbidity of type 2 diabetes and MDD group.
