| Objective:The current situation of patient trust in China is not optimistic,and there are many reasons that lead to patients’subjective distrust of doctors.Among them,patients’perception of their own diseases may be one of the important influencing factors.As an independent medical system,traditional Chinese medicine has a unique way of diagnosis and treatment,and the impact of disease perception on physician trust by patients in traditional Chinese medicine hospitals may have its own specificity.Therefore,this study aims to explore the relationship between disease perception and physician trust among patients in TCM hospitals,providing theoretical support for the harmonious relationship between traditional Chinese medicine clinical doctors and patients.Methods:Using the convenient sampling method,the patients who were treated in the Affiliated Hospital of Changchun University of traditional Chinese medicine and the Third Affiliated Hospital of Changchun University of traditional Chinese medicine from June2021 to December 2022 were selected as the subjects.The self-made general demographic questionnaire,Wake Forest Doctor Trust Scale(WFDTS),Inter-group Anxiety Scale(IA),Intolerable Uncertainty Scale(IU-12)and Brief Illness Perception Questionnaire(BIPQ)were used for the survey.In the first study,SPSS26.0 was used to analyze the possible risk factors of patients’physician trust by linear regression.In the second study,the methods of item analysis,exploratory factor analysis,confirmatory factor analysis and reliability test were used,and SPSS26.0 and Amos28.0 were used to test the reliability and validity of the Chinese version of Intolerable Uncertainty Scale(IU-12)among patients.In the third study,Processv3.5 plug-in in SPSS26.0 was used to test the adjustment effect of unbearable uncertainty,and a simple slope analysis was carried out in Excel.Results:(1)the results of item analysis showed that the difference of intolerable uncertainty between high and low groups was significant(p<0.001),and the total correlation coefficient was between 0.427 and 0.682(p<0.001).In the results of exploratory factor analysis,the value of KMO was 0.840 and the result of Bartlett sphericity test wasχ~2=1906.182(p<0.001),indicating that the data of this sample was suitable for exploratory factor analysis.Referring to the gravel map,three factors with characteristic roots greater than 1 were obtained,and the contribution rate was 56.118%.The optimal oblique method was adopted for rotation,and the results showed that the factor loads of the 12 items were all greater than 0.4(0.542-0.826),and the commonality of each item was greater than 0.3(0.329-0.703).The modified model fitting result in confirmatory factor analysis is as follows:χ~2=3.133,CFI=0.981,GFI=0.981,RIQ=0.034,TLI=0.949,SRMR=0.034,NFI=0.062 PNFI=0.548,PCFI=0.555.The Cronbach’sαand split-half reliability of the total scale were 0.829 and 0.883.The Cronbach’sαof the three subscales was 0.741,0.664 and 0.668,and the split-half reliability was 0.792,0.712 and0.736,respectively.(2)The total score of WFDTS in this study was 37.40±5.17,intergroup anxiety of patients(B=0.252,p<0.001)and overall evaluation of doctors(B=0.125,p<0.001)were important influencing factors of physician-patient trust,of which intergroup anxiety was a risk factor and overall evaluation of doctors was a protective factor.(3)There was a significant negative correlation between patients’disease perception and physician trust(r=0.134,p<0.001).Intolerance of uncertainty played a moderating effect on the relationship between patient’s disease perception and physician trust(p<0.001).The results of simple slope analysis showed that the predictive effect of disease perception on physician trust in the high intolerable uncertainty group(β=-0.449,p<0.001)was higher than that in the low intolerable uncertainty group(β=-0.605,p<0.001).Conclusion:(1)The IUS-12 has good psychometric characteristics in the Chinese patient and can be used as a tool to evaluate the intolerable uncertainty level.(2)Lower intergroup anxiety and higher overall evaluation of doctors are conducive to the establishment of good physician-patient trust.(3)The disease perception of patients with high uncertainty can strongly predict the trust of doctors,that is,disease perception has a greater impact on the trust,and it is easier to reduce the trust of patients. |
