| Objective:To observe the effect of muscle relaxant spasmodic manipulation combined with rehabilitation exercise on neck movement function in children with congenital muscular torticollosis,and to evaluate the efficacy and superiority of muscle relaxant spasmodic manipulation combined with rehabilitation exercise in the treatment of CMT.To seek evidence for whether surface electromyography can be used as an evaluation method for neck motor function in CMT children.Methods:A randomized controlled trial was used to divide 66 children into observation group and control group,with 33 cases in each group.There was no significant difference in gender and age between the two groups.The observation group was treated with muscle relaxant spasmodic manipulation combined with rehabilitation exercise,and the control group was treated with muscle relaxant spasmodic manipulation.The two groups were treated with muscle relaxant spasmodic manipulation 5 times a week,while the observation group was treated with rehabilitation exercise 2 times a day,10 times a week,1 month as a course of treatment.A total of 1 course of treatment was given.The symptom and signs scores,SCM thickness difference and thickness ratio,and sternocleidomastoid muscle electromyography signal before and after treatment were observed and compared.Results:A total of 62 children completed the treatment,including 32 in the observation group and 30 in the control group.The specific statistical results were as follows:1.Comparison of each item before treatment: there were no significant differences in general data(gender,age,disease site),symptom score,SCM thickness difference and thickness ratio,sternocleidomastoid muscle electromyography signal between the two groups(P > 0.05),indicating that the data of the observation group and the control group were comparable.2.Intra-group comparison before and after treatment: there were significant differences in symptom and signs scores,SCM thickness difference and thickness ratio,and sternocleidomastoid electromyography signal before and after treatment between the two groups(P < 0.05),indicating that the treatment methods of the two groups were effective for CMT children.3.Comparison between groups after treatment: After treatment,the total score of symptoms and signs,the difference value and thickness ratio of SCM thickness,and the difference value of sternocleidomastoid muscle electromyography signal in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P < 0.05),indicating that the observation group was better than the control group in improving CMT,and the observation group was significantly better than the control group in improving the neck rotation function of children.The facial symmetry of the two groups was improved,but there was no statistical difference.4.Comparison of overall efficacy: the total effective rate of the observation group was 90.63%,and that of the control group was 80.00%,the difference was statistically significant(P < 0.05),indicating that the overall efficacy of the observation group was better than that of the control group.Conclusion:1.muscle relaxant spasmodic manipulation is effective in the treatment of CMT.2.Compared with the muscle relaxant spasmodic manipulation,muscle relaxant spasmodic manipulation combined with rehabilitation exercise has more advantages in improving the neck movement function of children.It is an effective and feasible treatment scheme,which is worthy of clinical promotion.3.Surface electromyography be used as a clinical evaluation method for the neck movement function of CMT children. |
