| Purpose:The purpose of this study is to observe the clinical effect of balance electroacupuncture combined with thoracic major muscle release manipulation and ordinary electroacupuncture in the treatment of subacromial impingement syndrome(SIS),and compare the observation indexes of efficacy of the two treatments before and after treatment, including the performance criteria for shoulder joint disease treatment(JO A)and the changes of dynamic ultrasonic impingement grade of shoulder joint. To analyze and evaluate the effectiveness of balance electroacupuncture combined with pectoralis major muscle release manipulation in the treatment of shoulder impingement, and to provide ideas for clinical treatment of shoulder impingement syndrome. Methods:A total of 7 2 patients with shoulder impact syndrome who met the inclusion criteria were selected and grouped by random number method, and randomly divided into the "balance electroacupuncture combined with pectoral major muscle release manipulation" group(hereinafter referred to as the experimental group)and the" ordinary electroacupuncture" group(hereinafter referred to as the control group)with 36 cases each, regardless of gender. The control group was treated with ordinary electroacupuncture, and the experimental group was treated with balance electroacupuncture combined with thoracic major muscle release on the basis of acupoint selection in the control group. The two groups were treated once a day for 5 days a week, with rest on weekends for 3 weeks. Before and after treatment, the two groups of patients were scored using the shoulder joint disease treatment Achievement criteria(JOA)and the shoulder joint dynamic ultrasonic impingment grade to evaluate the treatment effect, and the adverse reactions and adverse events occurred during the experiment were recorded.Results:1.Comparison of baseline data:Before treatment, gender, age and injury of patients in the two groups were all(P>0.05)except age, which could be compared between groups.2.Evaluation criteria for the treatment of shoulder joint disease(JOA):After the treatment, the total JOA scores of the experimental group and the control group were increased to varying degrees,all(P<0.05),which was statistically significant. The scores of various factors(pain, function, movements of daily life, upward lifting, external rotation, internal rotation and joint stability)were compared between the two groups, and all of them were improved to varying degrees before treatment, with statistical significance(P<0.05),while X-ray changes had no significant changes. There was no significant difference in JOA assessment between the experimental group and the control group(P>0.05).This may be the reason why the sample size is too small, which can be confirmed by further large sample study.Dynamic ultrasonic impingement grading of shoulder joint:After the end of treatment, the scores of dynamic ultrasonic impingement grading of shoulder joint by balance electroacupuncture combined with pectoralis major muscle release technique and ordinary electroacupuncture increased in different degrees, with statistical significance,P<0.05.The difference was statistically significant. There was no significant difference between the experimental group and the control group(P>0.05).This may be the reason why the sample size is too small, which can be confirmed by further large sample study.Clinical efficacy evaluation:After treatment, there were 5 cases in the control group,28 cases in the effective group and 2 cases in the ineffective group, with a total effective rate of 94.28%.In the experimental group,2 cases were effective,32 cases were effective and 1 case was ineffective. The total effective rate was 97.14%.The comparison of the overall efficacy between the control group and the experimental group was conducted by non-parametric test,(P<0.05),and the difference between the two groups was statistically significant.Conclusion:(1)Balance electroacupuncture combined with thoracolumpectoralis major release technique and ordinary electroacupuncture can effectively improve the pain, range of motion, shoulder joint stability and quality of life of SIS patients.Acupuncture has no advantage in improving X-ray morphology.(2)The treatment of subacromial impingement syndrome by balance electroacupuncture combined with thoracic major muscle release manipulation in the experimental group showed better overall efficacy than that in the ordinary electroacupuncture group, and the significant recovery rate was better than that in the ordinary electroacupuncture group. Both of the two methods have good efficacy in the treatment of shoulder impingement, but the balance electroacupuncture combined with pectoralis major release technique is more significant in the overall efficacy than the common electroacupuncture group, which is worthy of further clinical research。... |
