Objective:To observe the clinical efficacy of ultrasound-guided needle-knife accurate positioning in the treatment of subacromial impingement syndrome.Methods:According to the diagnostic criteria,inclusion criteria and exclusion criteria to choose114 patients with subacromial impingement syndrome who come from the orthopedic department of the People's Hospital Affiliated to Fujian University of Traditional Chinese Medicine from November 2018 to December 2019.All the patients are randomly divided into experimental group and control group,57 cases in each group.The patients in the experimental group were treated with needle knife release according to Van holsbeeck's rotator cuff ultrasound examination method and the needle knife point of "subacromial impingement syndrome" in 2006 edition of "basic and clinical needle knife medicine",and the points were taken:coracoid process point,greater tubercle point of humerus,anteromedial point of acromial peak,inferior border point of acromion.Once a week,three weeks as a course of treatment.The patients in the control group were treated with needle knife release according to the needle knife operation method of "subacromial impingement syndrome" in 2006 edition of "basic and clinical medicine of needle knife".The point and course of treatment were the same with the experimental group.The VAS score and the ROM of shoulder joint of the two groups before treatment,4 weeks after treatment and 12 weeks after treatment,the ultrasound image of the experimental group before treatment and 12 weeks after treatment,the safety index and adverse reactions in the treatment of the two groups and the UCLA score of the two groups 12 weeks after treatment were recorded and statistically analyzed.Before treatment,there were no significant differences between the two groups in gender,main part of disease,age and course of disease.Data in this research is processed by the software of SPSS21.0.Results:1.In the VAS score,before treatment and 4 weeks after the treatment,4 weeks after the treatment and 12 weeks after the treatment within the group showed significant differences(P<0.01).Show that the two kinds of treatment methods can significantly relieve shoulder pain;And there were significant differences between the two groups 4 weeks after the treatment and 12 weeks after the treatment(P<0.01).Show that ultrasound-guided needle knife accurate positioning in the treatment of subacromial impingement syndrome had a better effect to relieve shoulder pain.2.In the ROM of shoulder joint,before treatment and 4 weeks after the treatment,4weeks after the treatment and 12 weeks after the treatment within the group showed significant differences(P<0.01).Show that two kinds of treatment methods can significantly improve the ROM of shoulder joint.And there were significant differences between the two groups 4 weeks after the treatment and 12 weeks after the treatment(P<0.01).Show that ultrasound-guided needle knife accurate positioning in the treatment of subacromial impingement syndrome had a better effect to improve the ROM of shoulder joint.3.Interpretation of the ultrasound image:before treatment,the ultrasound image of the experimental group showed that the impact sign of subacromial impingement syndrome was positive.12 weeks after the treatment,the ultrasound image of the experimental group did not show the impact sign of subacromial impingement syndrome.Show that ultrasound-guided needle knife accurate positioning release can effectively eliminate the factors that cause the subacromial impingement syndrome.4.During the treatment,there were no adverse reactions in both groups.Show that the safety of the two groups was ensured.5.According to the UCLA score results of 12 weeks follow-up in the two groups,49 cases were actually observed in the experimental group:43 cases were controlled cured,3 cases were markedly effective,2 cases were effective,1 case was invalid,8 cases were lost in the follow-up(total effective rate 98.0%,missing rate 14.0%);47 cases were actually observed in the control group:34 cases were controlled cured,3 cases were markedly effective,6 cases were effective,4cases were invalid,10 cases were lost in the follow-up(total effective rate 91.5%,missing rate17.5%);The difference between the curative effect ratio of the two groups was statistically significant(P<0.05).Show that ultrasound-guided needle knife accurate positioning in the treatment of subacromial impingement syndrome had better satisfaction and overall effect.Conclusions:Ultrasound-guided needle knife accurate positioning in the treatment of subacromial impingement syndrome can significantly relieve shoulder pain,improve the ROM of shoulder joint,ensure the safety of treatment,and gain better satisfaction.
