Research purpose:1.Understand the use and demand of type 2 diabetes patients for diabetes management APP.2.Understand the electronic health literacy of type 2 diabetes patients, the cognition and willingness to use diabetes management APP,and analyze its influencing factors.3.Analyze the impact of type 2 diabetes patients’ e-health literacy and diabetes management APP cognition on diabetes management APP use intention.Materials and Methods:From October 2021 to May 2022,570 patients with type 2 diabetes were surveyed in three tertiary general hospitals in Sichuan Province using a purposive sampling method.A cross-sectional survey and research method was adopted.By reviewing relevant domestic and international literature, the current status of domestic and international research and relevant theoretical models were collated, based on the Technology Acceptance Model(TAM),Perceived Risk Theory(PR)and health e-health literacy as the theoretical basis. The general information questionnaire of diabetic patients, the questionnaire of diabetic patients’ awareness and willingness to use diabetes APP and the e-health literacy scale were used to investigate the willingness to use and factors influencing type 2 diabetic patients, and the results were analysed using SPSS 22.0 and AMOS 23.0 software for data analysis.Result:1.Among 559 patients,17(3%)had used diabetes management APP.The main reason for non use was that 481 patients(88.7%)did not understand the diabetes management APP,followed by 120 patients(22.1%)who did not use the APP.More than half of the patients hope that the diabetes management APP can provide health programs for 289 people(51.70%),consulting doctors for 287 people(51.34%),multiple reminders for 279 people(49.91%),and learning about diabetes for 278 people(49.73%).Among the six latent variables of the willingness to use model, the willingness to use has the highest score and the electronic health literacy has the lowest score.Total Cronbach’s using willingness questionnaire ɑ The coefficient is 0.944,and Cronbach’s of each dimension ɑ The coefficient is between 0.831-0.963,which is greater than 0.8.2.Multiple linear regression analysis showed that gender, age, education level, family monthly income per capita and smartphone proficiency were the common influencing factors of e-health literacy(P<0.05).Smartphone proficiency is an influential factor on perceived usefulness(P<0.05).Age and family monthly income per capita were the influencing factors on perceived ease of use(P<0.05).Gender,age and smart phone proficiency were the influencing factors on attitude and willingness to use(P<0.05).3.Structural equation model analysis found that perceived ease of use and electronic health literacy positively affected perceived usefulness, with path coefficients of 0.511 and 0.115 respectively(P<0.05);Electronic health literacy has a positive impact on perceived ease of use,and the correlation path coefficient is 0.659(P<0.05);Perceived usefulness and perceived ease of use have a positive impact on the use attitude, and the relevant path coefficients are 0.546 and 0.380 respectively(P<0.05);Perceived usefulness and use attitude positively affect the intention to use,and the relevant path coefficients are 0.137 and 0.825 respectively(P<0.05).The perceived usefulness of perceived security and electronic health literacy to perception was not verified(P>0.05).Conclusions:1.Diabetes management APP,a new disease management model, has not been used by most type 2 diabetes patients, but their willingness to use it is at a medium to high level.2.The top four functional requirements of diabetes management APP for type 2diabetes patients are that doctors regularly provide health programs, consult doctors, have multiple reminder functions and learn about diabetes.3.Patients with different demographic characteristics have different degrees of influence on electronic health literacy, diabetes management APP cognition and use intention. The younger the patient and the more skilled the smartphone are,the higher the score of e-health literacy, perceived usefulness, perceived ease of use, attitude and willingness to use is;The higher the education level, the higher the score of electronic health literacy and perceived ease of use;The higher the monthly income per capita of the family, the higher the score of electronic health literacy, willingness to use and perceived ease of use;Male patients had higher scores of perceived ease of use, electronic health literacy, use attitude and use intention.4.Factors influencing the willingness of type 2 diabetes patients to use diabetes management APP include perceived ease of use, perceived usefulness,e-health literacy and attitude to use.Perceived ease of use, perceived usefulness, and electronic health literacy positively affect the use attitude, thus positively affect the use intention through the use attitude.5.Perceived safety of type 2 diabetes has no effect on willingness to use.
