| Objective: To compare the value of magnetic resonance imaging(MRI)and computed tomography(CT)in diagnosis and identification of spinal hydatid disease,and analyze its imaging signs.Methods: Ninety-eight patients with suspected spinal hydatid disease admitted to the hospital were collected between January 2010 and January 2022 and all were examined by CT and MRI.Taking the pathological results as the gold standard,compare the diagnostic efficacy of CT and MRI for spinal hydatid(include diagnostic accuracy,sensitivity,specificity,etc.),observe and analyze their CT and MRI imaging features,and discuss the value of CT and MRI in the diagnosis and differential diagnosis of spinal hydatid.Results: Among the 98 patients,33 were diagnosed as spinal hydatid,including 18 thoracic hydatid,4 lumbar hydatid,and 7 sacral hydatid,1thoracolumbar and 3 lumbosacral vertebrae involved.The CT showed that there were 26 cases with spinal hydatid.Compared with the golden standard,its sensitivity,specificity,accuracy and Kappa value were 78.79%,89.47%,78.79% and 0.687,respectively,AUC=0.841[95%CI:0.747-0.935].MRI showed that there were 29 cases with spinal hydatid.Compared with the golden standard,its sensitivity,specificity,accuracy and Kappa value were 87.88%,92.98%,87.88% and 0.809,respectively,AUC=0.904[95%CI:0.829-0.98].CT showed that 33 cases had bone destruction,including 16 cases of cystic expansive bone destruction,7 cases of osteolytic bone destruction,and 10 cases of irregular bone destruction,68 vertebrae in total.The vertebral accessory were involved in17 cases,ribs were invaded in 14 cases,spinal canal were invaded in 11 cases,paravertebral soft tissue were invaded in 27 cases,calcification in 12,and 4 cases with obvious narrowing of intervertebral space.MRI showed bone destruction in all 33 cases,including 19 cases of cystic expansive bone destruction and 6 cases of osteolytic bone destruction,62 vertebrae in total.Spinal canal was involved in 19 cases,intervertebral disc was involved in 3 cases,and paraspinal soft tissue was involved in 30 cases.The detection rate of bone destruction and calcification by CT was higher than that by MRI(P<0.05).MRI had a higher detection rate than CT in both disc involvement and spinal canal involvement(P<0.05).Conclusion: CT and MRI have certain clinical efficacy in the diagnosis of spine hydatid.CT has higher sensitivity to the bone destruction and calcification,while MRI has obvious advantages in the diagnosis of intervertebral disc involvement and spinal canal invasion.Both methods have their own advantages and disadvantages,and the combination of the two methods can provide more comprehensive imaging information in the clinic. |
