Objective: 1.Through the prevalence survey,to explore the status of delirium and its influencing factors in postoperative ICU patients,and determine the physical restraint as the risk factor of delirium,which provided a theoretical basis for subsequent intervention.2.Through the intervention studies,the effects of restraint alternative measures on the incidence of delirium,the rate of physical restraint,time of physical restraint and unplanned extubation were observed,and to explore its application effect in postoperative ICU patients.Method: 1.Using convenient sampling method,180 postoperative ICU patients in a tertiary hospital in Xinjiang were admitted to the study subjects,and the data were collected using the general data questionnaire,CAM-ICU,APACHEⅡ,and PSQI,PNI.2.In this study,postoperative patients admitted to ICU from June to July 2022 as the control group,and postoperative patients admitted to ICU from August 2022 to January2023 as the intervention group.The control group received routine nursing,and the intervention group was given restraint alternative measures on the basis of routine nursing.The incidence of delirium,the rate of physical restraint,time of physical restraint,and incidence of unplanned extubation was compared between the two groups.Results:1.There were 46 patients with delirium in 180 subjects,and the incidence of delirium was25.6%.Logistic multivariate regression analysis showed that APACHEⅡ score,operation time,physical restraint and sleep quality were independent risk factors for delirium in postoperative ICU patients.2.There was no significant difference in baseline data between the two groups before intervention(P>0.05);after intervention,there were significant differences in the incidence of delirium,the rate of physical restraint,time of physical restraint between the two groups(P<0.05),while the incidence of unplanned extubation between the two groups was not statistically significant(P>0.05).Conclusion: 1.The incidence of delirium in postoperative ICU patients is high,high APACHEⅡ score,long operation time,use of physical restraint and sleep disorder are independent risk factors for delirium.2.Restraint alternative measures can effectively prevent delirium of postoperative ICU patients,reduce the rate of physical restraint and time of physical restraint,and do not increase the incidence of unplanned extubation while intervening,ensuring the safety of patients.
