Observation On The Treatment Of Nasal Septum Deviation Combined With Hypertrophic Rhinitis With Different Surgical Schemes Under Nasal Endoscope

ObjectiveTo explore the effect of different surgical schemes in the treatment of nasal septum deviation combined with hypertrophic rhinitis.MethodsFrom December 2020 to January 2023,60 patients with deviated nasal septum and chronic hypertrophic rhinitis who were treated in our hospital were randomly divided into four groups:group A:submucosal resection of inferior turbinate+conventional suture of nasal septum deviation correction;group B:plasma mucosal ablation of bilateral inferior turbinate+conventional suture of nasal septum deviation correction;group C:surgical method:submucosal resection of inferior turbinate+continuous suture of nasal septum deviation correction,Group D was treated with plasma mucosal ablation of bilateral inferior turbinates+continuous correction of deviation of nasal septum.SNOT-22 score,nasal resistance test,and oxidative stress level were measured before,7 days after,1 month after and 3 months after operation,respectively,and the postoperative adverse conditions and treatment satisfaction were statistically analyzed.ResultsAlthough the preoperative scores of the four groups were relatively similar,the SNOT-22 scores of patients in group C and group D were significantly lower than those in group A and group B(P<0.05)since the first week after operation,and the difference lasted until three months after operation,but there was no significant difference between group C and group D(P>0.05).There was no significant difference in the preoperative nasal resistance of the four groups,and there was no significant difference in the nasal resistance one week after the operation.The difference between 1 month and 3 months after operation was statistically significant(P<0.05).The nasal resistance of group C and group D was significantly lower than that of group A and group B.And the difference between 3 months after operation and 1 month after operation was further increased(Z value),but there was no significant difference between group C and group D.According to the statistics of the adverse conditions of the four groups of patients after surgery,there was 1 case of nasal septum hematoma in Group A and 1 case of dry rhinitis in Group B.Generally,there was basically no adverse situation,and there was no significant difference between the groups.The postoperative satisfaction of the four groups was compared.Although the satisfaction of Group C and Group D was higher than that of Group A and Group B,the difference was not statistically significant(P>0.05).The levels of oxidative stress before and after operation were compared among the four groups.The levels of CAT,SOD and GSH were lower before operation.The levels of antioxidant enzymes in the four groups were lower before operation.The average level of antioxidant water in the four groups after operation was higher than that before operation.The average level of antioxidant water in D was the highest in the first week,the first month and the third month after operation,followed by Group C,except that there was no significant difference in GSH in the first week after operation,the other differences were significant(P<0.05).There was no statistically significant difference in the time of saccharin test before and after operation between the four groups(P>0.05),and there was no statistically significant difference in the time points within the group(P>0.05).ConclusionFor patients with nasal septal deviation combined with chronic hypertrophic rhinitis,there is no significant difference in the comparative efficacy between plasma mucosal ablation and submucosal turbinectomy.However,compared with conventional suture,continuous nasal septal suture can reduce nasal resistance and SNOT-22 score,improve the level of nasal antioxidant stress,and thus improve the postoperative quality of life of patients to a certain extent.
