| Objective:Based on data mining and network pharmacology methods,this study studied the prescriptions and syndromes of Huangqi Guizhi Wuwu Decoction.in order to explore the indications of Huangqi Guizhi Wuwu Decoction in modern clinical medical records,the symptoms,tongue pulse,drug dosage and clinical addition or reduction,as well as the relationship between symptoms and flavated drugs.For the two high frequency western diseases identified by data mining,network pharmacology was used to explore the mechanism of Huangqi Guizhi Wuwu Decoction on them.Methods:The medical cases of Huangqi Guizhi Wuwu decoction from August 1959 to November 2022 were collected from CNKI,VIP,WANFANG DATA,CBM and other databases.According to the inclusion and exclusion criteria,the medical records that met the inclusion criteria were selected and entered into Excel to establish a data set.Frequency analysis was performed on the basic information of patients,names of Chinese and western medicine diseases,symptoms,tongue and pulse,drug dosage and flavor.SPSS Modeler 18.0 was used to analyze the association rules of high-frequency symptoms and high-frequency flavored traditional Chinese medicine.SPSS Statistics 26.0 was used for cluster analysis of high frequency symptoms and high frequency flavated traditional Chinese medicine.The top two high-frequency western medicine diseases and Huangqi Guizhi Wuwu decoction were analyzed by network pharmacology.TCMSP database and BATMAN-TCM database were used to search the active ingredients of Huangqi Guizhi Wuwu decoction.PubChem and Swiss TargetPrediction database were used to predict the target genes of Huangqi Guizhi Wuwu decoction.GeneCards and OMIM databases were used to search the target genes related to rheumatoid arthritis and peripheral neuritis,and Venny2.1.0 was used to map drug targets and disease targets.The STRING platform and Cytoscape3.8.2 software were used to draw the PPI network diagram and drug-component-target-disease diagram,and the core active components and key proteins were obtained by network topological parameter analysis.GO function and KEGG pathway enrichment analysis were performed for the drug-disease intersection targets,and Autodock and PyMOL were used for molecular docking.Results:(1)A total of 374 cases were included in the preliminary analysis.Most of the cases were female,most of them were 20-40 years old,and most of them were diagnosed and treated in winter.(2)There were 58 kinds of TCM diseases in 374 cases,including arthralgia,postpartum pain,exanthema,etc.According to the name of the disease,it is divided into 13 categories,mainly limb meridian disease syndrome,gynecological disease syndrome,and cerebral system disease.A total of 109 western medicine disease names were involved,mainly rheumatoid arthritis,peripheral neuritis,autonomic nerve dysfunction,etc.According to the system,the names of western medicine diseases were divided into 11 categories,mainly nervous system diseases,rheumatic immune diseases,and motor system diseases.(3)The 374 cases involved 138 kinds of symptoms.The common symptoms were fatigue,numbness,dull complexion,wind and cold,body pain,sweating,poor appetite,fatigue,etc.,involving 43 kinds of tongue texture,27 kinds of tongue coating,and 83 kinds of pulse.Among them,light tongue and reddish tongue were the common tongue texture,thin white coating,and white coating was the common tongue coating.The common pulse patterns were thick,thin and weak pulse.A total of 9 groups were obtained by symptom association analysis.A total of 8 clusters were obtained by symptom cluster analysis.(4)The dosage of Huangqi Guizhi Wuwu decoction included in the medical records was commonly used in modern clinical practice,and most of them were shared by the original prescription drugs.There were 190 kinds of modified Chinese herbs,and the most frequently used were Angelica sinensis,Licorice,Jixueteng,Fengfeng,Chuanxiong Rhizoma,Atractylodes rhizoma,etc.According to their efficacy,they can be divided into 17 categories,among which the drugs for tonifying deficiency and promoting blood circulation and removing blood stasis are the most common.A total of 4 items were obtained from the association analysis of flavated traditional Chinese medicine,and 8 clusters were obtained from the cluster analysis of flavated traditional Chinese medicine.(5)Network pharmacology collected 62 active ingredients of Huangqi Guizhi Wuwu Decoction,including 208 component targets,3671 rheumatoid arthritis related targets,2899 peripheral neuritis related targets,and 54 common targets of drug components and diseases.Among them,the key active ingredients of Huangqi Guizhi Wuwu decoction in the treatment of rheumatoid arthritis and peripheral neuritis are kaempferol,quercetin,isorhamnetin,β-sitosterol,γ-sitosterol,stigmasterol,etc.The core targets are AKT1,VEGFA,EGFR,STAT3,MMP9,PTGS2,etc.KEGG pathway analysis showed that the treatment of rheumatoid arthritis and peripheral neuritis with Huangqi Guizhi Wuwu Decoction mainly involved EGFR tyrosine kinase inhibitor resistance,PI3K-Akt signaling pathway,HIF-1 signaling pathway and other pathways.Molecular docking results showed that the key active components of Huangqi Guizhi Wuwu decoction had strong docking activity with the core targets.Conclusion:1.Huangqi Guizhi Wuwu Decoction is widely used in the treatment of many diseases in internal medicine,gynecology and surgery of traditional Chinese medicine,which can correspond to diseases in many systems of modern western medicine.2.The common clinical symptoms,tongue and pulse of Huangqi Guizhi Wuwu decoction are consistent with the pathogenesis of qi deficiency and blood stagnation and Ying-wei diathesis.3.In modern clinical practice,Huangqi Guizhiwuwu decoction is mostly shared with the original prescription,and is often used in combination with tonic deficiency drugs and drugs for promoting blood circulation and removing blood stasis.4.The pharmacological mechanism of Huangqi Guizhi Wuwu decoction in the treatment of rheumatoid arthritis and peripheral neuritis may be achieved by kaferol,quercetin,isorhamnetin and other components acting on AKT1,VEGFA,EGFR and other targets through EGFR tyrosine kinase inhibitor resistance,PI3K-Akt signaling pathway,HIF-1 signaling pathway and other pathways. |
