Study On Quality Of Life And Its Influencing Factors In Cancer Patients From The Perspective Of Health Ecology

Objective: Based on the theory of health ecology,this study explored the factors influencing the quality of life of cancer patients from the perspectives of individual traits,behavioral characteristics,interpersonal network,living and working conditions,and policy environment,and provided suggestions for improving the quality of life of cancer patients in Anhui province.Methods: Social demographic characteristics(such as gender,age,place of residence,education,bmi,marital status,etc.),economic status(such as family annual income,etc.),behavioral characteristics(such as smoking,drinking,insomnia,exercise,etc.),clinical characteristics(such as cancer staging,disease course,metastasis,etc.)of cancer patients in Anhui Province were described.The description includes the number of cases,constituent ratio,mean value,and standard deviation;a T-test or one-way analysis of variance were used if the conditions of normal distribution and homogeneity of variance were met.Man-Whitney U test or Kruskal-Wallis H test in the non-parametric test were used if the conditions of homogeneity of variance and normal distribution were not met.Tobit regression was used to analyze the factors affecting the quality of life of cancer patients and to explore the influence of related factors on the quality of life of cancer patients.Binary Logistic regression was adopted to analyze the five sub-dimensions of the quality of life of cancer patients and explore the influence of related factors on the five sub-dimensions.Results: Based on the theoretical model of health ecology,the quality of life of cancer patients was affected at five levels,and the influencing factors of personality traits were comorbidities,course of the disease,and metastasis.At the behavioral level,the influencing factors were smoking,insomnia,stress,exercise,depression,anxiety,delayed hospitalization,and management literacy.At the interpersonal network level,the influencing factors were family-instrumental support,social-emotional support,residence,education level,and cancer knowledge and attitude.Working and living conditions were affected by annual household income and hospitalization environment.The influence factor of the health policy layer is serious disease insurance.From the perspective of health ecology,the influencing factors of the sub-dimensions of the quality of life of cancer patients were analyzed.In the dimension of action ability,the influencing factors were: exercise,depression,management literacy and serious disease insurance;In the dimension of self-care,the influencing factors were: metastasis,depression and critical illness insurance;In the dimension of daily activities,the influencing factors were: gender,comorbidity,insomnia,management literacy,family-tool support,annual household income,exposure to harmful substances,hospitalization environment and serious illness insurance;The factors influencing the pain/discomfort dimension were: insomnia and anxiety;Factors influencing the depression/anxiety dimension were: clinical stage,course of disease,surgery,and socio-economic support.Conclusion and suggestions: In general,the life quality of cancer patients in Anhui Province is at a low level,among which,the behavioral characteristics layer and interpersonal network layer have a greater impact on the life quality of cancer patients in Anhui Province.From the perspective of each sub-dimension,the proportion of patients with difficulty in anxiety/depression was the highest,followed by pain/discomfort,daily activities,mobility,and self-care,and there were many influencing factors in each dimension.It is suggested to pay attention to the individual characteristics of patients and strengthen self-management;Strengthen health education to promote healthy behavior;Enrich personal network and provide social support;Optimize the hospital environment and strengthen the help;Improve medical insurance policies to enhance multi-dimensional well-being,to improve the quality of life of cancer patients.
