Correlation Between TyG Index And Coronary Artery Stenosis In Patients With Type 2 Diabetes Mellitus And Coronary Heart Disease

Abstract/Summary:
Objective:To study the correlation between serum triglyceride glucose product index(TyG)and the degree of coronary artery stenosis in patients with coronary atherosclerotic heart disease(CHD)and type 2 diabetes(T2DM),so as to facilitate early intervention and provide early treatment for patients with CHD and type 2 diabetes.Methods:A total of 365 patients who were hospitalized in the Department of Cardiology,Affiliated Hospital of Chengde Medical College from February2021 to February 2022 and met the inclusion criteria were selected as the research objects.According to the prevalence of CHD and T2 DM,they were divided into CHD group,T2 DM group and CHD combined with T2 DM group,including 156 cases in CHD group,82 cases in T2 DM group and 127 cases in CHD combined with T2 DM group.The baseline data and laboratory examination of the three groups were recorded,and the TyG index was calculated.The clinical data of the three groups were compared,and P < 0.05 was considered statistically significant.According to the Gensini score median method,the CHD combined with T2 DM group was divided into high score group and low score group.Multivariate logistic regression was used to analyze the correlation between TyG index and the degree of coronary artery stenosis in CHD patients with T2 DM.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of related risk factors in predicting the degree of coronary artery stenosis in CHD patients with T2 DM.Results:1.2.Comparison of clinical data and laboratory examination among the three groups : There were significant differences in hypertension history,smoking history,TyG index,fasting blood glucose,total cholesterol,low density lipoprotein,creatinine and urea nitrogen among the three groups(P< 0.05).2.There were significant differences in smoking history,hypertension history,TyG index,fasting blood glucose,triglyceride and total cholesterol between the low Gensini score group and the high Gensini score group(P < 0.05).3.Multivariate Logistic regression analysis of the severity of coronary artery stenosis(Gensini score)in CHD patients with T2 DM showed that hypertension history,smoking history,TyG index and total cholesterol were risk factors for the severity of coronary artery stenosis.4.ROC curve was used to analyze the predictive efficiency of TyG index on the severity of coronary artery stenosis in CHD patients with T2 DM.The results showed that in CHD patients with T2 DM,the area under the ROC curve AUC was 0.723(95 % CI 0.613-0.815,P < 0.001),the best cut-off value was 10.036,the sensitivity was 71.4 %,and the specificity was 73.4 %.5.Multivariate logistic regression analysis showed that TyG index was an independent risk factor for the degree of coronary artery stenosis in CHD with T2 DM,which was positively correlated with Gensini score,and its r value was 0.469(P < 0.001).Conclusions:1.TyG index was positively correlated with type 2 diabetes mellitus combined with coronary artery stenosis.
