The Expression And Clinical Significance Of P53, Ki-67, TOPOⅡα In Endometrial Carcinoma

Abstract/Summary:
Objective:By detecting the expression levels of P53,Ki-67 and TOPOⅡα in endometrial cancer tissues,the relationship between P53,Ki-67,TOPOⅡαand clinicopathological characteristics of endometrial cancer and the correlation between the expression of P53,Ki-67 and TOPOⅡα in endometrial cancer tissues were analyzed and the application value of combined detection of P53,Ki-67 and TOPOⅡα in the diagnosis of high-grade endometrial cancer were analyzed.To provide an effective basis for the prognosis and individualized treatment of patients with endometrial cancer.Methods:Ninety-five patients with endometrial cancer treated in the Department of Gynecology of Handan Central Hospital from June 2019 to June 2022 were selected as the experimental group,and 30 patients with proliferative endometrium during the same period were selected as the control group.IHC was used to detect the expression of P53,Ki-67 and TOPOⅡα in 125 cases.SPSS26.0 statistical software was used to analyze the relationship between P53,Ki-67,TOPOⅡα and clinicopathological features of endometrial carcinoma,and the correlation of the expression of the three markers in endometrial cancer,and to analyze the value of P53,Ki-67,TOPOⅡα in the diagnosis of high-grade endometrial carcinoma.Results:1.The positive expression rates of P53,Ki-67 and TOPOⅡα in endometrial cancer were 48.4%,72.6%,80.0%,which was significantly higher than 13.3%,23.3%,26.7% in proliferative endometrium.There was significant difference between the two groups(P < 0.05).2.There was no significant difference in the expression of P53 in the age of onset(P > 0.05).There were significant differences in the expression of P53 in tumor type,histological grade,surgical stage,depth of myometrial invasion,vascular tumor thrombus and lymph node metastasis(P < 0.05).3.There was no significant difference in the expression of Ki-67 between the age of onset and tumor type(P > 0.05).Ki-67 expression was significantly correlated with histological grade,surgical stage,depth of myometrial invasion,vascular invasion,and lymph node metastasis(P <0.05).4.There was no significant difference in the expression of TOPOⅡα in the age of onset,tumor type and histological grade(P > 0.05).There were significant differences in the expression of TOPOⅡα in surgical stage,depth of myometrial invasion,vascular invasion and lymph node metastasis(P <0.05).5.Spearman correlation analysis showed that the expression of P53 was positively correlated with Ki-67 in 95 cases of endometrial carcinoma(r=0.588,P < 0.001).The expression of P53 was positively correlated with that of TOPOⅡα(r =0.446,P < 0.001).The expression of Ki-67 was positively correlated with that of TOPOⅡα(r =0.542,P < 0.001).6.The AUC value of combined detection of P53,Ki-67 and TOPOⅡαin the diagnosis of high-grade endometrial carcinoma was 0.883,and the AUC values of P53,Ki-67 and TOPOⅡα in the diagnosis of high-grade endometrial carcinoma were 0.814,0.803 and 0.718.The value of combined detection in the diagnosis of high-grade endometrial cancer is higher than that of single detection.Conclusions:1.The high expression of P53,Ki-67 and TOPOⅡα in endometrial carcinoma suggests that they are closely related to the malignant biological behavior of endometrial carcinoma.2.P53,Ki-67 and TOPOⅡα synergistically promote the occurrence and development of endometrial cancer.3.The combined detection of P53,Ki-67 and TOPOⅡα can be used as a reliable index for the diagnosis of high-grade endometrial carcinoma.4.Combined detection of P53,Ki-67 and TOPOⅡα can accurately reflect the development process of endometrial cancer,and provide reliable basis for the prognosis evaluation and individualized diagnosis and treatment of patients.
