| Objective:This study was conducted to clarify the clinical efficacy of Kangfuxin liquid on postoperative wound healing in Crohn’s disease anal fistula patients by applying Kangfuxin liquid to the wound irrigation and filling Vaseline gauze containing drugs for dressing change after hanging drainage,observing the Crohn’s disease-related scores,wound healing and detecting relevant inflammatory factors and cell growth factors in patients,and investigating its mechanism of action.Methods:Fifty-two patients with perianal fistulizing Crohn’s disease(PFCD)who were hospitalized and operated at the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from January 2021 to December 2022 were screened,and all patients with PFCD were divided into two groups by random sampling method:Kangfuxin liquid group(26 cases)and control group(26 cases),both groups were operated by hanging drainage and treated with a regular course of biologics(infliximab)after surgery.Dressing change started on the 2nd day after surgery(twice a day);During dressing change operation,Kangfuxin liquid group was first given Kangfuxin liquid to flush fistulas and wounds,and then filled with Vaseline gauze containing medicinal liquid for drainage.The control group was given 0.9% normal saline dressing change,the same method as Kangfuxin liquid group,and then Vaseline gauze was used to fill the drainage.Observe and record before and after treatment patient Crohn’s disease activity index(CDAI),perianal Crohn’s disease activity index(PDAI),anal defecation function score(Wexner score),wound healing and test related cell growth factor FN(Fibronectin),EGF(Epidermal growth factor);inflammatory factor IL-6(Interleukin-6),TNF-α(Tumor necrosis factor-α),and the data collected from the study were processed using statistical software(spss23.0)and analyzed for the study.Results:1.the general data of gender,age,preoperative CDAI score,PDAI score,BMI value and Wexner score were comparable between the two groups(P>0.05).2.Comparison of CDAI scores and PDAI scores: At 1 month and 2 months after treatment,CDAI scores decreased with treatment time in both groups(P<0.05),and the difference in scores between groups was not statistically significant(P>0.05),and there was no significant difference in the decreasing trend(P>0.05).The differences of time effect,group effect and time×group interaction effect of PDAI scores between the two groups were statistically significant(P<0.05),and it was known by simple effect analysis that the PDAI scores of patients in both groups decreased after treatment,and the decrease was more obvious in the Kangfuxin liquid group compared with the control group.3.Comparison of Wexner scores: There was no significant difference between the Wexner score and the preoperative comparison and groups at 2 months(P>0.05).4.Comparison of serum FN and EGF levels: There was no statistically significant difference in the expression levels of serum FN and EGF between the two groups at 1 d before surgery(P>0.05);the expression levels increased at half a month after surgery compared with 1 d before surgery(P<0.05),and the increase in the expression levels of serum FN and EGF was more significant in the Kangfuxin liquid group(P<0.05).5.Comparison of the levels of inflammatory factors IL-6 and TNF-α: The differences in the expression levels of inflammatory factors IL-6 and TNF-α between the two groups were not statistically significant at 1 d before surgery(P>0.05);the expression levels decreased at half a month after surgery compared with 1 d before surgery(P<0.05),and the decrease in the expression levels of inflammatory factors IL-6and TNF-α was more significant in the Kangfuxin liquid group(P <0.05).Conclusion:The use of Kangfuxin liquid for wound dressing change after PFCD hanging drainage,based on the application of basic biological agent(infliximab)treatment,effectively reduced the perianal activity of the disease and promoted the increase of FN and EGF expression levels,down-regulated the postoperative inflammatory factors IL-6and TNF-α levels to reduce the inflammatory response of the body,and accelerated the healing of the postoperative fistula wound. |
