Abstract/Summary:
Objective:The Montreal Cognitive Assessment Scale(MoCA)was used to evaluate the neurocognitive impairment caused by different causes,and to explore the clinical value of the total score and its sub-items in neurocognitive impairment.Method:This study is a retrospective analysis.A total of 513 patients with neurocognitive impairment in the outpatient and inpatient department of neurology in The Third Bethune Hospital of Jilin University from December 2020 to September 2022 were collected.Among them,173 patients met the inclusion criteria.According to the etiology,the enrolled patients were divided into Alzheimer’s disease(AD)group(n=62),vascular dementia(Va D)group(n=59)and Parkinson’s disease dementia(PDD)group(n=52);All subjects were tested with MoCA and MMSE;The differences between groups were analyzed for the total score of MoCA and sub-item scores of patients in each group,and the correlation between the differences and MMSE scores in each group was analyzed.Results:1.There were differences in visual space and executive ability(P=0.001),delayed recall(p=0.003)and directional ability(p=0.001)between AD group and PDD group;There were differences in visual space and executive ability(p=0.001),attention(p=0.007)and orientation ability(p=0.001)between AD group and Va D group;There was a difference in attention between Va D group and PD group(p=0.002).2.The scores of visual space and executive ability in Va D group(r=0.312,p=0.016)and PDD group(r=0.395,p=0.004)showed a low positive correlation with MMSE scores.3.There was a low positive correlation between attention score and MMSE score in AD group(r=0.350,p=0.005)and PDD group(r=0.359,p=0.009).4.Delayed recall score was positively correlated with MMSE score in AD group(r=0.664,p<0.001)and PDD group(r=0.503,p<0.001).5.The score of directional ability in AD group(r=0.314,p=0.013),Va D group(r=0.360,p=0.005)and PDD group(r=0.419,p=0.002)showed a low positive correlation with MMSE score.Conclusions:MoCA sub-item score has significant difference among patients with cognitive impairment in AD,Va D and PDD groups.Different sub-items have different correlation with the three groups of diseases,which has certain clinical guiding significance for the identification of etiology and rehabilitation treatment.
