| Objective:(1)To understand the current situation of primary caregiver burden,psychological capital and coping style after hip fracture surgery in the elderly,and to analyze the differences in general data;(2)To explore the correlation of primary caregiver burden,psychological capital and coping style after hip fracture surgery in the elderly;(3)To analyze the main influencing factors of the burden of primary caregivers after hip fracture surgery in the elderly,so as to reduce the burden level of primary caregivers and provide theoretical basis for proposing effective intervention measures.Method : This study was a cross-sectional investigation,and convenience sampling method was adopted.During 2022.01 to 2022.12,300 elderly patients and their main caregivers after hip fracture surgery were conveniently selected from the orthopedic ward of two third-class A hospitals in Yan ’an,Shaanxi Province,and questionnaire data were collected.The survey tools included general data of patients and primary caregivers,caregiver burden Scale(ZBI),Positive Psychological Capital Questionnaire(PPQ),and Simple Coping Style Scale(SCSQ).Statistical software SPSS25.0 was used for data analysis in this study,and statistical methods such as frequency,percentage,mean and standard deviation,one-way analysis of variance,independent sample t test,Pearson correlation analysis and multiple linear regression analysis were selected.Results:(1)Primary caregiver burden after hip fracture in elderly patientsIn this study,the burden level of primary caregivers after hip fracture in the elderly was moderate(61.0%),and the total score was(42.29±14.53).The caring burden of the study subjects had statistically significant differences in the reasons of injury,types of surgery,chronic diseases,self-care ability,age,education level,marital status,per capita monthly family income,health status of the main caregiver,living with the patient,relationship with the patient,average daily caring time,and help from others(P < 0.05).(2)Psychological capital of primary caregivers after hip fracture in elderly patients.In this study,the total score of psychological capital of the main caregivers after hip fracture surgery was(116.34±13.09)points.The average score of optimism dimension was the highest(4.91±0.62)points,while the average score of toughness dimension was the lowest(4.08±0.54)points.In addition,there were significant differences in the psychological capital of the main caregivers in gender,age,education level,per capita monthly family income,working status,relationship with patients,and average daily care time(P < 0.05).(3)Coping styles of primary caregivers after hip fracture in elderly patients.In this study,the score of positive coping was higher than that of negative coping among the primary caregivers after hip fracture surgery.The total score of coping style was(32.08±8.46),positive coping style score was(19.84±5.40)and negative coping style score was(12.23±4.12).In addition,the total scores of coping styles of the main caregivers were statistically significant in the caregiver’s age,education level,per capita monthly family income,relationship with patients,average daily caring time,whether they lived together,and with or without help from others(P < 0.05).There were statistically significant differences in the caregiver’s age,education level,working status,average monthly income of family members,average daily caring time,whether they lived together,and whether they had help from others(P < 0.05).There were statistically significant differences in the main caregivers’ negative coping in work status,average monthly income of family members,relationship with patients,health status,daily care time,whether they lived together,and whether they had help from others(P < 0.05).(4)Correlation of primary caregiver burden,psychological capital and coping styles after hip fracture surgery in elderly patients.In this study,primary caregiver burden,psychological capital and coping styles were correlated with hip fracture surgery in elderly patients.The burden of primary caregivers was negatively correlated with the self-efficacy dimension and the resilience dimension of psychological capital(P < 0.01).Primary caregiver burden was negatively correlated with coping style and positive coping(P < 0.01),and positively correlated with negative coping(P < 0.01).Coping style was positively correlated with psychological capital,self-efficacy,hope and optimism(P < 0.01),and positive coping was positively correlated with psychological capital(P < 0.01)。Negative coping was negatively correlated with psychological capital(P < 0.01),self-efficacy(P < 0.01),resilience(P < 0.05)and optimism.(5)Multiple linear regression analysis of primary caregiver burden after hip fracture in elderly patients.The regression results showed that six variables,including chronic disease,caregiver age,average monthly income of family members,resilience,positive coping and negative coping,entered the multiple regression equation,with R2=0.531 and adjusted R2=0.503.Therefore,meaningful variables could jointly explain 50.3% variation in the burden of primary caregivers.Conclusions:(1)The burden of primary caregivers after the operation of hip fracture in the elderly was more moderate;The score of optimism dimension was the highest and the score of toughness dimension was the lowest among the main caregivers after hip fracture surgery.The primary caregivers after hip fracture in the elderly mainly adopt a positive coping approach to face the caregiver burden.(2)The main caregiver burden,psychological capital and coping style were correlated.The higher the psychological capital level and positive coping score,the lower the caregiver burden.(3)The main influencing factors of primary caregiver burden after hip fracture surgery include chronic disease,caregiver age,average monthly family income,resilience, positive coping,negative coping,etc. |
