Objective:Based on the general understanding of patients with rheumatoid arthritis and the main caregivers of the family,to investigate the level and status of self-perceived burden, coping style and the caregiver's family burden, analyzing the factors affecting the patient's self-perceived burden,to explore the correlation between the patient's self-perceived burden, coping style and the caregiver's family burden, and to provide a research basis for promoting the physical and mental health of patients and caregivers. Methods:In this study,a convenient sampling method was used to select 198 RA patients and major family caregivers from the Department of Rheumatology and Immunology,a hospital of 3grade in Lanzhou City,Gansu Province, from March 2018 to November 2018.Research tools include the General Information Questionnaire, the self-perceived Burden Scale, the medical coping style Scale, and the disease family burden scale. After numbering the data, use the Excel form to enter the data. The statistical software SPSS22.0 is used to analyze the data.Statistical methods include descriptive statistical analysis,t-test,one-way ANOVA,multiple linear stepwise regression, pearson correlation,etc.Results:(1)The average score of self-perceived burden of patients with rheumatoid arthritis is24.99±10.11 points, the body burden dimension score is 11.92±5.01 points, the emotional burden dimension score is 9.90±4.19 points, and the economic burden dimension score is 3.17±1.34points;The patients of yield coping dimension score was 11.57±3.52 points, which was statistically significant compared with the domestic norm(P<0.05);Among the family burdens of the caregiver, the positive rate of daily household activities was the highest(98.0%),and the positive rate of mental health of family members was the lowest(31.3%).(2)The patients with rheumatoid arthritis of DAS28 score, yield yield coping style and the caregiver's family financial burden are the main influencing factors of the body burden dimension of the self-perceived burden scale; The patientsof current living status,C-reactive protein,DAS28 score, the yield coping style, the caregiver's job type and family economic burden, which is the main influencing factor of the emotional burden dimension; The patients of monthly medical expenses, the yield coping style, the caregiver's family total annual income and family economic burden, which is the main influencing factors of the economic burden dimension; The patients of monthly medical expenses, personal monthly income, the current living conditions,C-reactive protein,DAS28 score, the yield coping style, the caregiver's family total annual income and family economic burden, are the main factors affecting the total score of the self-perceived burden scale.(3)The total score of self-perceived burden scale of patients with rheumatoid arthritis was negatively correlated with the face dimension of the medical coping modes quesionnaire(P<0.01),positively correlated with avoidance and yield dimension(P<0.01),and positively correlated with the total score of the caregiver's family burden scale of diseases(P<0.01);The total score of the caregiver's family burden scale of diseases was positively correlated with the medical coping modes quesionnaire's avoidance and yield dimension score(P<0.01).Conclusions:In this study, the self-perceived burden of patients with rheumatoid arthritis was at a moderate level, and the influencing factors were mainly related to monthly medical expenses, personal monthly income, current living status,C-reactive protein,DAS28 score, the yield coping style, the caregiver's family total annual income and family economic burden. In clinical practice,we should pay attention to and correctly assess the self-perceived burden of patients with rheumatoid arthritis, promote communication between patients and family members, reduce the pressure of care, encourage patients to make full use of social resources to reduce the economic burden, and help guide patients and their families.Actively respond to diseases, encourage patients to participate in medical decision-making and community chronic disease management, and help patients recover as soon as possible.
