Objective: In this project,we analyse the TCM evidence elements of young and middle-aged patients with essential hypertension,discuss and summarise their distribution characteristics,and explore the correlation between the distribution of TCM evidence elements and ambulatory arterial stiffness index in young and middle-aged patients with essential hypertension,so as to provide a clinical basis for the diagnosis and treatment of young and middle-aged patients with essential hypertension in TCM.Methods: Two hundred and twelve young and middle-aged patients with hypertension aged between 18 and 64 years who were hospitalized in the Department of Cardiovascular Medicine of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from September 2021 to September 2022 were collected and subjected to 24-hour ambulatory blood pressure monitoring,inclusion and exclusion criteria were established,a Chinese medical evidence questionnaire for primary hypertension in young and middle-aged people was created,and the four diagnostic information data were initially quantified and then established An Excel database was created and statistical analysis was performed using SPSS 26.0.Result: 1.212 cases were included in this study,115(54.25%)were males and 97(45.75%)were females.2.81 cases(38.21%)were found to be mono-pathological,111(52.36%)were bi-pathological and 30(9.43%)were tri-pathological.The majority of the single-site evidence was liver(26.89%)and the majority of the mixed-site evidence was liver-kidney syndromes in 37cases(33.04%),followed by spleen-kidney syndromes in 30 cases(26.79%)and liver-spleen syndromes in 27 cases(24.11%)in the order of >20%.It is suggested that hepatorenal co-morbidity is the main pathological and miscellaneous evidence in young and middle-aged hypertensive patients.3.By frequency statistical analysis,there were 80 cases(37.74%)of single pathological evidence,94 cases(44.34%)of double pathological evidence and38 cases(17.92%)of triple pathological evidence.The single deficiency pathology was mostly Yin deficiency in 18 cases(8.49%),and the deficiency and miscellaneous pathology was mainly Qi and Blood deficiency in 3 cases(1.42%).The AASI ≥ 0.55 was considered as the abnormal value,and by chi-square test,the incidence of abnormal AASI was higher in blood stasis and phlegm-damp pathologies than in the other pathologies(P<0.05).Binary logistic regression analysis showed that age,smoking history,alcohol history,body mass index,total cholesterol,triglycerides and fasting glucose were statistically significant(P<0.05)for the common TCM evidence in young and middle-aged hypertensive patients,with age positively correlated with renal evidence;smoking history positively correlated with blood stasis and hyperactive yang evidence;alcohol history positively correlated with spleen and phlegm-damp evidence;and body mass index positively correlated with spleen and phlegm-damp evidence.The correlation between body mass index and spleen,blood stasis and phlegm-dampness evidence was positive;total cholesterol and evidence of liver and hyperactivity were positively correlated;triglycerides and evidence of kidney were positively correlated;fasting blood glucose and evidence of kidney were positively correlated.Conclusion: The distribution of disease location and disease nature in young and middle-aged patients with essential hypertension was most common with dual disease location and disease nature,with most of the disease location evidence elements being in the liver,most of the solid disease evidence elements being blood stasis,and most of the deficiency disease evidence elements being yin deficiency;most of the disease location inclusions were liver and kidney co-morbidities,most of the solid inclusions were qi stagnation and blood stasis,most of the deficiency inclusions were qi and blood deficiency,and most of the deficiency inclusions were yin deficiency and yang hyperactivity.2.Among the TCM evidence elements in 212 young and middle-aged patients with essential hypertension The incidence of abnormal AASI indices was higher for the phlegm-damp and blood-stasis evidence elements compared with other pathological evidence elements.3.There was a correlation between the common TCM evidence elements of essential hypertension in young and middle-aged patients and the biochemical indices and general conditions of the patients.
