Objective: To observe the clinical effect of Yiqi Runchang Recipe on Qi deficiency syndrome in children with functional constipation,and to study the effect of Yiqi Runchang Recipe on Qi deficiency constipation in rats through animal experiments,and to preliminarily explore its possible mechanism,so as to provide a new therapeutic regimen for clinical treatment of Qi deficiency syndrome in children with functional constipation.Methods: Clinical study: 60 cases of functional constipation with Qi deficiency syndrome were randomly divided into treatment group(n = 30)and control group(n = 30).The treatment group was given Yiqi Runchang Decoction orally,and the control group was given lactulose orally.Both groups were treated for 14 days.Observe and record the TCM syndrome scores of the two groups of children before and after treatment,fill in the clinical observation table,perform statistical analysis,evaluate the clinical efficacy and explore its mechanism of action.Experimental study:48 healthy male SD rats were randomly divided into blank group,model control group,lactulose group,low dose Yiqi Runchang Decoction group,medium dose Yiqi Runchang Decoction group and high dose Yiqi Runchang Decoction group,8 rats in each group.The blank group was given normal saline by gavage every day,the other five groups were given loperamide suspension by gavage every day to construct constipation model,and the model of Qi deficiency was established by the method of excessive tiredness and hunger-satiety disorder.The model lasted for 14 days.At the end of modeling,the number of fecal pellets within 24 hours was recorded and weighed wet in each group,and the dry weight of feces after drying was recorded to calculate the water content.After the model was successfully established,the rats in each group were given drug intervention,the blank group and the model control group were given normal saline by gavage every day;the rats in the low,medium and high dose groups of Yiqi Runchang Decoction were given 4.375 g/(kg · d),8.75 g/(kg · d)and 17.5 g/(kg · d)by oral gavage of Yiqi Runchang Decoction for suspension,respectively;the rats in the lactulose group were given 1.67 g/(kg · d)lactulose solution by gavage.Daily gavage for 14 days.At the end of drug intervention,the protein expression of SCF,c-kit and AQP3 in rat colon tissues was detected by HE staining and Western blot.Results: Clinical study: The total effective rate was 96.7% in the treatment group and 90% in the control group.The two groups had similar efficacy,and the difference had no statistical significance(P > 0.05).In improving the total score of main symptoms,the two groups had similar efficacy(P > 0.05),and in improving the total score of secondary symptoms and the total score of syndromes,the treatment group was superior to the lactulose group(P < 0.05).The two groups had similar efficacy in improving stool characteristics,frequency of defecation,defecation,abdominal pain and abdominal distension,feeling of incomplete defecation,and fatigue after defecation(P > 0.05),and the treatment group had better efficacy in improving spontaneous sweating and fatigue(P <0.05).Experimental study: After the end of modeling,the diet and water intake of rats in each model group decreased,the body weight decreased(P< 0.05),the size of feces was uneven,the number of fecal particles decreased(P < 0.05),the wet weight decreased(P < 0.05),the movement was quiet,the spirit was poor,and the exercise endurance decreased,showing qi deficiency performance.After drug intervention,rats in each group of Yiqi Runchang Decoction and lactulose group recovered their diet and drinking water,gained weight,had uniform stool particle size,and increased range of motion.In terms of fecal wet weight,compared with the blank group,the fecal wet weight of rats in the model control group decreased,and the difference was statistically significant(P < 0.05);compared with the model control group,the fecal wet weight of rats in the medium dose group of Yiqi Runchang Fang and the high dose group of Yiqi Runchang Fang increased,and the difference was statistically significant(P < 0.05).In terms of dry fecal weight,compared with the blank group,the dry fecal weight of rats in the model control group decreased,and the difference was statistically significant(P < 0.05);compared with the model control group,the dry fecal weight of rats in the medium dose group of Yiqi Runchang Fang and the high dose group of Yiqi Runchang Fang increased,and the difference was statistically significant(P < 0.05).In terms of fecal water content,there were no significant differences between the groups in pairwise comparisons(P > 0.05).In terms of the number of fecal pellets,compared with the blank group,the dry weight of feces decreased in the model control group,and the difference was statistically significant(P < 0.05);compared with the model control group,the dry weight of feces increased in the low-dose group of Yiqi Runchang Fang,the medium-dose group of Yiqi Runchang Fang,and the high-dose group of Yiqi Runchang Fang,and the difference was statistically significant(P <0.05).HE staining of colon in each group showed that the epithelial cells in the mucosal layer were intact and neatly arranged and regular,the glandular structure was normal,and no inflammatory cell infiltration was observed.HE staining showed no significant difference between the groups,indicating that functional constipation was not an organic lesion,and the structure of colon tissue did not change significantly.The expression of c-kit,SCF and AQP3 protein in the model control group was lower than that in the blank group,and the expression of c-kit,SCF and AQP3 protein in each group of lactulose and Yiqi Runchang Decoction was increased compared with the control group.Conclusion: Yiqi Runchang Recipe has a good therapeutic effect on Qi deficiency syndrome of functional constipation in children,without adverse reactions and is safe to use.It is worthy of being widely popularized and applied.The experimental study showed that Yiqi Runchang Recipe could improve the general condition of rats with Qi deficiency constipation,improve the wet weight of feces,dry weight of feces and the number of defecation pellets,confirming the therapeutic effect of Yiqi Runchang Recipe on rats with Qi deficiency constipation,and the results of related protein expression showed that the mechanism of Yiqi Runchang Recipe in treating functional constipation may be related to its repair of SCF/c-kit signaling pathway.
