| Objective: Based on the traditional Chinese medicine theory of "physique and spirit theory ",this study aims to explore the efficacy and safety of Bupleurum Liver-soothing and Mind-Tranquilizing Decoction in treating stable angina pectoris(stagnation of vital energy and blood stasis type)combined with depression.The goal is to provide a more effective traditional Chinese medicine intervention method for clinical diagnosis and treatment of patients with "dual heart diseases".Method: This clinical study adopted a randomized,simple,and controlled method,including 64 eligible patients(32 in the control group and 32 in the treatment group).Both groups of patients were given conventional western medicine treatment recommended by the guidelines,and psychological counseling.In addition,patients in the treatment group were given Bupleurum Liver-soothing and Mind-Tranquilizing Decoction for28 days.The changes in Traditional Chinese Medicine(TCM)syndrome score,angina pectoris attack frequency,Zung Self-Rating Depression Scale(SDS)score,Athens Insomnia Scale(AIS)score,plasma arteriosclerosis index(AIP),and safety indicators before and after treatment were recorded for both groups.Software was used for comparative analysis to evaluate its clinical efficacy and safety.Results:1.Baseline data: There was no statistically significant difference(P>0.05)between the two groups in terms of baseline data such as gender,age,and average duration of illness,indicating comparability between the groups.2.TCM syndrome integral:Intracomparison showed that the quantified scores of traditional Chinese medical(TCM)syndrome evaluations of both groups decreased significantly after treatment(P<0.01).Intercomparison showed that the quantified scores of TCM syndrome evaluations of the treatment group were lower than those of the control group after treatment,with a significant difference(P<0.01).In terms of the total effective rate of TCM syndrome,the treatment group was 90.63%,and the control group was 75.00%,with a significant difference between the two groups(P<0.05).3.Angina attack situation: Intra-group comparison showed that the quantitative score for angina attacks after treatment in both groups(including the number of attacks,duration,and nitroglycerin usage)decreased significantly compared with before treatment(P<0.01).Intergroup comparison showed that the quantitative score for angina attacks after treatment in the treatment group was significantly lower than that in the control group(P<0.01).Regarding the overall effective rate of angina treatment,it was 90.63% in the treatment group and 78.13% in the control group,with the treatment group being higher than the control group(P<0.05).4.SDS integral: Within-group comparison showed that the SDS scores of both groups of patients were significantly reduced after treatment compared to before treatment(P<0.01).Between-group comparison revealed that the SDS scores of the treatment group were significantly lower than those of the control group after treatment(P<0.01).The total effective rate for treating depressive disorders was 75.00% in the treatment group and 56.25%in the control group,with the treatment group being significantly higher than the control group(P<0.05).5.AIS score: Intra-group comparison showed that the AIS scores of patients in the treatment group decreased significantly after treatment compared to before treatment(P<0.01),while there was no significant difference in AIS scores before and after treatment in the control group(P>0.05).Inter-group comparison showed that the AIS scores of the treatment group after treatment were significantly lower than those of the control group(P<0.05).6.AIP index: Within-group comparison showed that the AIP index of patients in the treatment group decreased significantly after treatment compared to before treatment(P<0.01);the AIP index of patients in the control group also decreased significantly after treatment compared to before treatment(P<0.01)).Between-group comparison showed no significant difference in the AIP index between the two groups after treatment(P>0.05).Conclusion:1.Bupleurum Liver-soothing and Mind-Tranquilizing Decoction can effectively improve the TCM syndrome of patients with stable angina pectoris(stagnation of vital energy and blood stasis)and coexisting depression,reduce the frequency of angina attacks,and improve their quality of life.2.Bupleurum Liver-soothing and Mind-Tranquilizing Decoction can significantly improve patients’ depressive symptoms,alleviate negative emotions,and improve sleep quality.3.Bupleurum Liver-soothing and Mind-Tranquilizing Decoction can effectively reduce the AIP index of patients,but the difference in efficacy compared with conventional lipid-lowering drugs is not significant.4.The clinical application of Bupleurum Liver-soothing and MindTranquilizing decoction is safe and effective. |
