Explore The Academic Thoughts And Clinical Experience Of Prof.hou Aihua In The Treatment Of Gastric Cancer Based On Data Mining And Interview Methods

1.ObjectiveIn order to guide the clinical application,objectively study the case information of outpatient gastric cancer patients of Professor Hou Aihua,excavat the medication rules of Professor Hou Aihua in the treatment of gastric cancer and summary the experience of Professor Hou in the treatment of gastric cancer with the help of data mining software and the the interview method.2.MethodsFrom October 1,2019 to October 1,2022,Professor Hou Aihua’s outpatient diagnosis and treatment conformed to the standards of "Chinese Standards for the Diagnosis and Treatment of Common Malignant Tumors"(published in 1991,the fourth volume)and the 2011 edition of "Gastric Cancer TCM Diagnosis and Treatment Plan(Trial)",and after data screening in strict accordance with the inclusion criteria and exclusion criteria,361 medical cases of 216 pat ients were obtained and entered into the EXCEL worksheet.After standardiza tion,the database was entered into the TCM inheritance assistance platform(V2.5),and the information of patients’ gender,age distribution,and the four properties,five tastes,meridian tropism,core prescription,and new prescri ption of commonly used drugs were obtained by using frequency statistics,as sociation rule analysis,entropy clustering and unsupervised entropy hierar chy clustering;Based on the pre-set interview outline,interview Professor Hou Aihua and several patients randomly selected from the database with semi-structured one-on-one face-to-face in-depth interview method,obtain the feedback of pat ients on Prof.Hou Aihua’s diagnosis and treatment of gastric cancer using Nvivo collation and analysis,and summary the clinical experience and academic thoughts of Prof.Hou Aihua in the treatment of gastric cancer based on the research above results.3.ResultsThe 361 prescriptions collected involved a total of 271 Chinese medicines,and the drugs used in the top 10 were:Tangerine peel,Fried Atractylodes mac rocephala,Poria cocos,Astragalus membranaceus,Pinellia ternate,Codonopsis pilosual,Medicated leaven,Fried licorice,Bletilla striata and Ligustrum luc idum.The four properties of medicine are mainly warm and cold,the five tastes are mainly sweet and bitter,and the meridian tropism are mainly the spleen,stomach and liver.Through data mining methods such as association rule anal ysis method,complex hierarchical entropy clustering analysis method,and unsu pervised entropy hierarchical clustering,obtain the common drug pairs,core drug combinations and new prescriptions applied to four treatment methods and six syndrome types.A total of 13 interviewees(12 patients and Professor Hou Aihua himself)were included in this interview study,12 patients had a male to female ratio of 1:1,an average age of 62 years old,an average interview length of about8 minutes,and a transcript of 6125 words,which were qualitatively analyzed by using Nvivo software,and 94 original codes,6 primary codes,4 secondary codes,and 1 tertiary code were obtained;The interview with Professor Hou las ted about 54 minutes,with 11,027 words of transcript,and was qualitatively analyzed by Nvivo software,and 74 original codes,20 primary codes,5 secondary codes,and 1 tertiary code were obtained.4.ConclusionsProfessor Hou Aihua believes that the weakness of the body and the invasion of evil qi are the basic pathogenesis of tumor formation,while the core path ogenesis of gastric cancer is spleen and kidney deficiency,asthenia of qi and blood,stagnation of liver qi,and endogenesis of phlegm and blood stasis.The treatment takes strengthening the body resistance to eliminate pathogenic factors as the core,and is equipped with regulating qi and removing blood stasis,invigorating the spleen and removing dampness,qi-blood reinforcing,clearing away heat and nourishing yin,invigorating the spleen and kidney,soothing liver and harmonizing stomach.In addition,when using clinical pre scriptions,we should pay attention to what kind of treatment methods patients use,and different treatment methods need to cooperate with different basic formulas to truly achieve the integration of traditional Chinese and Western medicine.
