| Objective:Stroke is a major chronic non-communicable disease that seriously endangers the health of Chinese residents.As the main type of stroke,The incidence and prevalence of ischemic stroke continue to grow.As a disease that can be prevented and controlled,The study of risk factors of ischemic stroke is the premise of effective intervention measures.Based on this,This article will use the case-control study to explore the risk factors of 803 cases of elderly ischemic stroke in Hohhot,and explore the gender differences of risk factors of ischemic stroke,In order to provide the basis for scientific and reasonable intervention measures.Methods: In this study,803 patients with ischemic stroke aged 60 years and above from three hospitals in Hohhot from September 2019 to June 2022 were included as the case group,and 1096 community non-stroke physical examination people aged 60 years and above from four communities in Hohhot during the same period were included as the control group,and all the researchers met the exclusion criteria.Epidemiological data were collected by questionnaire survey,Including past history,family history,physical examination,biochemical tests and so on.SPSS 25.0 software was used for statistical analysis to analyze the distribution of epidemiological data,past history,family history,physical examination and biochemical tests between the two groups.Multivariate Logistic regression analysis was used to explore the risk factors of ischemic stroke,and stratified analysis and sensitivity analysis were performed.Result: 1.Body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FBG),glycosylated hemoglobin(Hb A1c),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and uric acid in the case group were significantly lower than those in the control group;And smoking,drinking,hypertension,diabetes,dyslipidemia,coronary heart disease,family history of stroke were higher than the control group,while high density lipoprotein cholesterol(HDL-C)levels were lower than the control group,The difference was statistically significant(P < 0.05).2.Multivariate Logistic regression analysis showed that the risk of ischemic stroke was related to hypertension in descending order of odds ratio(OR: 6.94;95% CI: 5.47-8.80),diabetes(OR: 2.32;95% CI: 1.74-3.09),coronary heart disease(OR: 2.04;95% CI: 1.42-2.91),uric acid(OR: 1.70;95% CI: 1.53-1.89),age(OR: 1.13;95% CI: 1.11-1.16),diastolic pressure(DBP)(OR: 1.03;95% CI: 1.02-1.05),HDL-C(OR: 0.52;95% CI: 0.38-0.72).3.Multivariate Logistic regression analysis showed that the odds ratio(OR)of male ischemic stroke was significantly higher than that of hypertension(OR: 7.88;95% CI:5.79-10.72),diabetes(OR: 2.72;95% CI: 1.83-4.05),total cholesterol(OR: 1.39;95% CI:1.18-1.63),uric acid(OR: 1.32;95% CI: 1.14-1.52),Hb A1c(OR: 1.27;95% CI:1.09-1.47),age(OR: 1.11;95% CI: 1.08-1.14),diastolic pressure(DBP)(OR: 1.02;95%CI: 1.01-1.03).4.Multivariate Logistic regression analysis showed that the risk of ischemic stroke in women was significantly higher than that of hypertension(OR: 6.21;95% CI: 4.19-9.20),coronary heart disease(OR: 2.69;95% CI: 1.57-4.60),uric acid(OR: 2.42;95% CI:2.02-2.90),diabetes(OR: 1.95;95% CI: 1.26-3.03),age(OR: 1.16;95% CI: 1.13-1.21),diastolic pressure(DBP)(OR: 1.04;95% CI: 1.02-1.06),HDL-C(OR: 0.50;95% CI:0.27-0.93).Conclusion:In the study,According to the OR value from big to small,In turn is hypertension,diabetes,coronary heart disease,uric acid,age and diastolic blood pressure(DBP)level is the risk factors of ischemic stroke,and HDL-C level is the protective factors of ischemic stroke;Hypertension,diabetes,total cholesterol(TC),uric acid,glycosylated hemoglobin(Hb A1c),age and diastolic blood pressure(DBP)were the risk factors of male ischemic stroke;Hypertension,coronary heart disease,uric acid,diabetes,age and diastolic blood pressure(DBP)are risk factors of ischemic stroke in women,while high density lipoprotein cholesterol(HDL-C)is a protective factor of ischemic stroke in women;There were gender differences in risk factors of ischemic stroke in 803 elderly people in Hohhot,and the OR value of hypertension was the largest among all the risk factors. |
