Abstract/Summary:
ObjectiveTo investigate the relevant factors that affect the occurrence of lymph node metastasis in patients with vascular infiltration of colorectal cancer,and providing reference for predicting lymph node metastasis in vascular infiltration patients.MethodsAccording to the inclusion and exclusion criteria,a total of 162 patients admitted to our hospital for surgical treatment of colorectal cancer from January 1,2019 to December 31,2022 at the Third Affiliated Hospital of Southern Medical University were screened.Basic patient information was collected,including age,gender,degree of differentiation,type of differentiation,tumor diameter,lymph node metastasis,CEA,CA19-9,depth of infiltration,smoking,nerve infiltration,and other influencing factors,Divide vascular infiltration into lymph node metastasis group(LNI group)and non lymph node metastasis group(NLNI group),and use SPSS software 25.0,chi square test and logistic regression analysis statistical methods to explore the correlation between LNI group and NLNI group,and then identify the risk factors for lymphatic metastasis in vascular infiltration colorectal cancer patients.Results1.Comparing the population aged≥ 60 years and gender in the LNI group and NLNI group,there was no statistically significant difference after statistical analysis.2.The LNI group and NLNI group were grouped according to their degree of differentiation,with low differentiation groups accounting for 17.2%and 11.5%,respectively;The moderately differentiated group accounted for 72.1%and 72.4%respectively;The highly differentiated group accounted for 10.7%and 16.1%respectively;The differences between the groups were statistically significant through statistical analysis.3.In the general types of tumors in the LNI and NLNI groups,the infiltrative type accounted for 6.6%and 10.3%respectively,while the protruding type accounted for 40.8%and 49.2%respectively;Ulcerative type accounted for 52.6%and 40.5%respectively;After statistical analysis,there was no statistically significant difference between the groups.4.The proportion of cases with tumor diameter≥5cm in the LNI group and NLNI group was 52.6%and 46.3%,respectively.After statistical analysis,there was no statistically significant difference between the groups.5.The proportion of preoperative CEA values≥5ng/ml in the LNI group and NLNI group was 58.1%and 26.1%,respectively.After statistical analysis,the differences between the groups were statistically significant.The proportions of preoperative CA19-9 values≥35U/ml in the LNI group and NLNI group were 35.4%and 7.3%,respectively,with statistical significance.6.The T1 phase accounted for 6.4%and 11.6%of the LNI and NLNI groups,respectively;The T2 phase accounted for 7.5%and 13.1%respectively;The T3 phase accounted for 52.6%and 60.9%respectively;The T4 phase accounted for 33.5%and 14.4%respectively,and the differences between the groups were statistically significant.7.The proportion of smoking cases in the LNI group and NLNI group was 51.6%and 40.5%,respectively,and there was no statistically significant difference between the groups.The occurrence of nerve infiltration in the LNI and NLNI groups was statistically significant.8.Logistic regression analysis showed that differentiation degree(OR=1.745),infiltration depth(OR=1.242),and neural infiltration(OR=4.921)were independent risk factors for the LNI group and the NLNI group;Conclusion1.The depth of infiltration,degree of tumor differentiation,and neural infiltration are independent risk factors for lymph node metastasis in patients with vascular infiltration.2.The occurrence of lymph node metastasis in patients with vascular invasion of colorectal cancer is related to the degree of differentiation,CEA,CA19-9,nerve infiltration,and depth of tumor infiltration,but is not related to age,gender,general type,tumor diameter,or smoking.
