| Objective: The purpose of this study was to investigate the status of quality of life of esophageal cancer patients in Hulunbuir,Inner Mongolia,explore and analyze the influencing factors related to the quality of life,and put forward corresponding countermeasures and suggestions,so as to provide theoretical basis for improving the quality of life of patients with esophageal cancer and promoting their rehabilitation.Methods: From December 2021 to December 2022,a total of 126 patients diagnosed with esophageal cancer by pathological examination were continuously included in the outpatient and inpatient department of Inner Mongolia Forestry General Hospital,Hulunbuir Second People’s Hospital and Yakeshi People’s Hospital.Through descriptive research methods,data of all patients were analyzed,including general data,Quality of Life Assessment Scale(EORTC QLQ-C30),and esophageal cancer specific scale(QLQ-OES18).spss22.0 was used for statistical analysis;general data were described by percentage,frequency and mean;life quality of patients was described by mean ± standard deviation;t test and analysis of variance were used to analyze the influence of demographic characteristics and clinical disease characteristics on life quality of patients.Statistically significant factors were used as independent variables,and all dimensions of quality of life were used as dependent variables.Multiple linear regression analysis was performed.Results:1.A total of 130 questionnaires were sent out in this study,and 126 were effectively received,with effective recovery rate of 96.92%.2.Sociodemographic characteristics and clinical characteristics of patients with esophageal cancer:Sociodemographic characteristics: There were 126 patients in this group,including 119 males and 7 females;The average age was 69.25 years old(45-89 years old),11 cases were younger than 55 years old,32 cases were 55-65 years old,49 cases were 65-75 years old,34 cases were older than 75 years old;Ten patients lived alone and 116 lived with their families.Five patients were treated with medical insurance at their own expense,52 with medical insurance for urban residents and 69 with medical insurance for employees.Clinical disease characteristics: 30 cases were diagnosed less than 1 year,50 cases in 1 to 3years,28 cases in 3 to 5 years,11 cases in 5 to 7 years and 7 cases after 7 years;There were 13 cases in the upper segment,70 in the middle segment,and 43 in the lower segment.114 cases of squamous cell carcinoma and 12 cases of adenocarcinoma;There were 60 cases without metastasis,45 cases with lymph node metastasis and 21 cases with distant metastasis.There were 55 cases of stage 0 and stage Ⅰ,34 cases of stage II,14 cases of stage III,and 23 cases of stage IV.There were 10 cases of ESD,59 cases of radical operation,14 cases of simple radiotherapy,13 cases of chemotherapy combined immunity,18 cases of radiotherapy combined chemotherapy,and 12 cases of palliative or conservative treatment.3.Quality of life of patients with esophageal cancer: Physical function(62.28±14.39),role function(68.92±18.66),emotional function(63.36±12.83),cognitive function(63.36 ±21.60),social function(48.81±26.26),total health status(27.84±13.79)were lower than on Norwegia The n norm.Fatigue(38.71±18.79),nausea and vomiting(26.46±20.90),and pain(28.31±24.79)were all higher than the Norwegian norm.In the single item,the highest score of economic hardship was(55.82±33.16),followed by constipation(46.83±30.16)and diarrhea(26.19±26.87).4.Factors affecting the quality of life: Multiple linear regression analysis showed that age,gender and pathological stage were the main factors affecting the functional dimension.Gender,lesion site,payment method and metastasis were the main factors affecting symptom dimension.The mode of payment is the main factor affecting economic difficulties;Metastasis is a major factor affecting overall health status.Conclusion:1.The overall quality of life of esophageal cancer patients in Hulunbuir,Inner Mongolia was poor.Compared with the Norwegian norm,the physical function,role function,cognitive function,emotional function,social function and total health status were poor.Fatigue,nausea and vomiting,pain,insomnia,shortness of breath,loss of appetite,diarrhoea,constipation and financial difficulties were more pronounced.2.Esophageal cancer patients in Hulunbuir,age,gender and pathological stage were the factors affecting the functional dimension.Gender,lesion site,payment method and metastasis were the factors that affected symptom dimension.The mode of payment is the factor that affects economic hardship;Metastasis is a factor affecting overall health status. |
