Objective: To investigate the life quality of esophageal cancer patients in Shihezi,Xinjiang,analyze its possible influencing factors,and explore ways to improve life quality,so as to provide basis for formulating effective and targeted intervention mechanism in this area.Methods: A descriptive study was performed.150 esophageal cancer patients who was pathologically confirmed and had informed consent at the First Affiliated Hospital of Medical College of Shihezi University from January 1st,2012 to September 31 st,2017 were selected.Life quality of patients with esophageal cancer were assessed by EORTC QLQ-C30 and QLQ-OES18 developed by European Cancer Research and Treatment Organization.The influences of social demography,clinicopathological characteristics and other factors on life quality were analyzed by t-test,one-way ANOVA and multiple linear regression.Results:1.A total of 150 questionnaires were sent out in this study,and 148 valid questionnaires were collected.The valid rate was 98.7%.2.Social demographic and clinical characteristics of esophageal cancer patients General demographic characteristics: of 148 patients,107 males(72.3%)and 41 females(27.7%);aged43-91 years,with an average age of(68.45±9.38)years,of whom 15 were under 55 years old,39 were between 55 and 65 years old,55 were between 65 and 75 years old,39 were over 75 years old;134 were Han,10 were Kazakh,4 were other nationalities;8 live alone,140 were living with their families;9 were self-financed,98 of medical insurance and 41 cases of new agricultural cooperative medical system.Clinical features: 39 were diagnosed for less than 1 year,46 for 1-3 years,29 for 3-5 years,24 for 5-7 years;126 of squamous cell carcinoma and 22 of adenocarcinoma;112 without metastasis,26 of lymph node metastasis and 10 of distant metastasis;85 of stage 0,I,37 cases of stage II,17 of stage III,9 of stage IV;12 of ESD,81 of radical resection,36 of radiotherapy and chemotherapy,19 of conservative treatment.3.The life quality of esophageal cancer patients: life quality with esophageal cancer patients is poor.The scores of somatic function(83.29±15.68),role function(81.47±15.08),social function(73.31±13.71),total health status(71.90±13.44)in QLQ-C30 scale are lower than those of Norgian norm;the emotional function score was the highest(94.03±10.43)and the social function score was the lowest(73.31±13.71)in the functional field.In all symptom areas,except fatigue,pain and insomnia,the scores were higher than those of the norm.The higher scores were nausea and vomiting(5.18±11.80),shortness of breath(16.44±17.60),loss of appetite(8.56±15.12),constipation(17.79 ±20.73),diarrhea(9.68 ±18.75),dysphagia(17.79±20.73).The constipation score was higher(17.79±20.73)and the financial difficulty score was the highest(30.85±18.68),and the dysphagia score was the highest(27.50±17.72)in QLQ-OES18.4.Influencing factors of life quality: Multivariate linear regression analysis showed that the main influencing factors of total health status of patients with esophageal cancer were metastasis and years of diagnosis;the main influencing factors of functional dimension were age,living style,years of diagnosis,metastasis,pathological stages and treatment methods;the main influencing factors of symptomsdimension were age,nationality,years of diagnosis,metastasis and treatment methods;the main influencing factors of economic difficulties were income and types of health insurance.Conclusion:1.Life quality of patients with esophageal cancer is poor.Compared with the norm,the physical function,role function,social function and general health status are poor;nausea and vomiting,shortness of breath,loss of appetite,constipation,diarrhea,dysphagia symptoms and economic difficulties are more obvious.2.Income and type of medical insurance are the influencing factors of economic difficulties.Increasing investment in cancer prevention and treatment and the proportion of cancer reimbursement can alleviate the family and social and economic burden to a certain extent,thus improving the life quality of esophageal cancer patients.3.Age,living style,pathological stage and treatment are the main factors affecting the life quality of patients.Early screening,diagnosis,treatment and health education can improve the life quality of esophageal cancer patients.
