| Objective:Vertigo is one of the common symptoms in otolaryngology and neurology patients.Clinically,vertigo is classified into vestibular systemic vertigo and non-vestibular systemic vertigo.Benign Paroxysmal Positional Vertigo(BPPV)is the most common peripheral vestibular disease.The prevalence was 10.7-140 per 100,000 population,with an annual prevalence of 1.6%and an annual incidence of 0.6%.Some patients with BPPV can recover completely through manual reduction,but some patients will still have a second attack after recovery through manual reduction,and repeated vertigo will seriously affect patients’ daily life.Therefore,this study will make a preliminary model for predicting the recurrence of BPPV by looking for the risk factors of recurrent BPPV,so as to control the risk factors and reduce the recurrence through health education and other means,and provide references for clinicians in diagnosis and treatment of the disease.Methods:A total of 423 patients who were diagnosed with BPPV and underwent repositioning treatment in the Department of Otorhinolaryngology,Nanfang Hospital,Southern Medical University from October 2020 to June 2021 were collected.According to whether the patients recurred after 7 days of manual repositioning,they were divided into recurrence group(160 cases)and non-recurrence group(263 cases).The basic information(gender,age),past medical history(whether complicated with hypertension,diabetes and hyperlipidemia)and attack type(angiolithiasis/cupuolithiasis)when BPPV was first diagnosed were determined by telephone followup and previous medical records.SPSS26.0 statistical software was used to analyze and process the data.The single factor test was performed first,and the indicators with statistically significant differences(P<0.05)were analyzed by Logistic regression analysis again.Finally,the statistically significant indicators were determined,and the risk prediction model for predicting the recurrence of BPPV patients was designed to evaluate the recurrence of patients.Finally,the practicability of the model was verified.Results:There were significant differences in age,gender,hypertension,diabetes,and hyperlipidemia between the non-recurrence group and the recurrence group(P<0.05),and the above factors were all independent risk factors for BPPV recurrence.There was no significant difference in seizure type(angiolithiasis/cupulolithiasis)between the two groups(P>0.05).Conclusion:Gender,age,hypertension,diabetes,and hyperlipidemia can be used as risk factors to evaluate the recurrence of BPPV.The above can be included in the prediction model to design a preliminary prediction model for the short-term recurrence of BPPV.Therefore,patients can be scored at the first visit,and the possibility of recurrence can be reduced by lifestyle intervention and health education. |
