Objective: To observe the clinical characteristics of benign paroxysm positional vertigo(BPPV),and explore the factors affecting recurrence of BPPV, provide reference for clinical treatment.Methods: BPPV patients who were diagnosed and treated in The Affiliated Hospital of the Armed Police College from August 2012 to March 2015 were enrolled in our study. All patients were followed-up 12 months. The data of the patients with recurrence BPPV were recorded including sexuality, age, treatment, hypertension, Diabetes mellitus, hyperlipidemia, cervical spondylosis, head injury. And the comparison were made between the patients with BPPV recurrence and un-recurrence.The factors affecting recurrence rate of BPPV were studied by applying ?2 test and the Logistic regression.Results:1 The incidence of the Primary BPPV was 88.0%(288 cases), and the Secondary BPPV was12.0%(29 cases). The incidence of the Primary BPPV was higher than the Secondary BPPV. The clinical factors of sexuality, diseased ear, diseased semicircular canal between the Primary BPPV and the Secondary BPPV was different, which had statistical significance(P<0.05).2 After 1~3 times of canalith repositioning procedures, the remission cases of BPPV were 267, and the mean remission rate was 81.7%. The Primary BPPV remission cases was237, remission rate was 82.3% and the Secondary BPPV remission cases was 30, remission rate was 76.9%. After 12 months followed-up, 21 cases were loss including 18 cases of Primary BPPV and 3 cases of Secondary BPPV. 246 cases were effective cases, and 69 case had recurrence, the mean recurrence rate was 28.0%. The Primary BPPV recurrence cases was 51, recurrence rate was 23.3% and the Secondary BPPV recurrence cases was 18, recurrence rate was 66.7%. The recurrence cases of the Secondary BPPV was higher than the Primary BPPV, which had statistical significance(P<0.05).3 The data of the patients with recurrence BPPV were recorded including sexuality, age, hypertension, Diabetes mellitus, hyperlipidemia, cervical spondylosis, head injury, migraine, posterior circulation ischemia, severe cough, treatment method. And the comparison were made between the patients with BPPV recurrence and un-recurrence. The factors affecting recurrence rate of BPPV were studied by applying ?2 test and the Logistic regression. The result showed that the migraine, head injury, pure medication, hypertension and cough were independent risk factors of recurrence of BPPV,these factors had statistical significance(P<0.05).Conclusions:1 The Primary BPPV was more common than the Secondary BPPV, and the recurrence of the Primary BPPV was lower than the Secondary BPPV.2 Migraine, head injury, pure medication, hypertension and cough were independent risk factors of recurrence of BPPV. Clinical staffs should pay more attentions to these factors for newly diagnosed BPPV patients.
