Objective:Summarize the development and origin of Traditional Chinese Medicine(TCM)disease names for osteoporosis from ancient literature,compare and summarize the etiology,pathogenesis,treatment methods,and prescriptions for treating osteoporosis in ancient and modern literature.Analyze the medication patterns for treating osteoporosis in ancient and modern times,explore the similarities and differences in medication patterns for treating osteoporosis in ancient and modern times,and provide reference for clinical treatment of osteoporosis.Methods:Analyze the etiology,pathogenesis,treatment principles,methods,and treatment formulas of osteoporosis through literature research and data mining techniques.Collect and organize ancient and modern literature separately,and screen relevant formulas based on inclusion criteria.Using Excel 2019,SPSS Statistics 21,SPSS Modeler 18.0,and Cytoscape 3.9.1 software,frequency analysis,association rule analysis,and cluster analysis were performed on the prescription.Results:1.Literature research has found that the suitable TCM names for osteoporosis are“atrophic debility of bones”and“involving the bone”proposed during the Qin and Han dynasties,“bone exhaustion”appeared during the Sui and Tang dynasties,and“Bone sparsity”and“Bone shortening”mentioned during the Song,Jin,and Yuan dynasties.In addition,the theory of“bone erosion”mentioned by Li Dongyuan and the theory of“Bone void”recorded by Chen Shiduo are similar to current osteoporosis,but more doctors have different views,so they are not suitable as traditional Chinese medicine names for osteoporosis.The“Bone fragmentation”and“Bone fracture”recorded in the Xianshu Lishang Continuously Breaking Secret Recipe often represent traumatic fractures,and cannot be used as a traditional Chinese medicine name for osteoporosis.During the Qin and Han dynasties,literature focused on the main causes and pathogenesis of kidney essence deficiency,invasion of external pathogens,and the initiation of other diseases.The corresponding treatment principles and methods were to tonify kidney essence and eliminate external pathogens.During the Sui and Tang dynasties,attention was paid to the etiology and pathogenesis of liver and kidney deficiency,with nourishing the liver and kidney as the main treatment principle.During the Song,Jin,and Yuan dynasties,there were numerous literature documenting the etiology and pathogenesis of liver and kidney deficiency and spleen and stomach weakness.At this time,nourishing the liver and kidney and tonifying the spleen and stomach were the main treatment principles.During the Ming and Qing dynasties,based on summarizing previous experiences,attention was paid to the impact of abnormal Qi and blood circulation on osteoporosis.In addition to the above treatment principles and methods,attention is also paid to maintaining the normal operation of qi and blood,such as promoting qi and promoting blood circulation,and supplementing qi and blood.The etiology and pathogenesis mentioned in modern literature mainly include kidney essence deficiency,liver and kidney yin deficiency,spleen and stomach weakness,spleen and kidney yang deficiency,kidney deficiency and blood stasis,blood stasis and qi stagnation,etc.Corresponding treatment principles and methods are adopted,such as tonifying kidney essence,nourishing liver and kidney,tonifying spleen and stomach,warming spleen and kidney,tonifying kidney and activating blood,and promoting qi and blood circulation.2.Data mining involves 253 ancient prescriptions,involving 296 traditional Chinese medicines,884 modern prescriptions,and 284 traditional Chinese medicines.The frequently used drugs in ancient and modern formulas include Achyranthes bidentata,Eucommia ulmoides,Psoralea,Poria cocos,Angelica sinensis,Rehmannia glutinosa,Astragalus membranaceus,licorice,etc.The main effects of high-frequency drugs are:tonifying deficiency drugs,promoting blood circulation and resolving stasis drugs,and promoting water and dampness.Yang tonifying drugs are mainly used in tonifying deficiency.High frequency drugs are mostly warm and mild,with ancient high-frequency drugs accounting for 74.38%and modern high-frequency drugs accounting for 82.78%;The medicinal taste is mainly sweet,spicy,and bitter,with ancient high-frequency drugs accounting for 83.52%and modern high-frequency drugs accounting for 82.63%;The kidney meridian,liver meridian,and spleen meridian are the most common meridians,with ancient high-frequency drugs accounting for 60.48%and modern high-frequency drugs accounting for 71.75%.Association rule analysis:There are a total of 2 combinations associated with ancient prescription drugs,and 62 combinations associated with modern prescription drugs.Cluster analysis revealed that ancient clustering yielded 4 categories,while modern clustering yielded 4 categories.Conclusions:1.The traditional Chinese medicine name for osteoporosis should be:“atrophic debility of bones”,“involving the bone”,“bone exhaustion”,“Bone sparsity”,and“Bone shortening”.2.Both ancient and modern literature have identified kidney essence deficiency,liver and kidney yin deficiency,and spleen and stomach weakness as the main causes and pathogenesis,with tonifying kidney essence,nourishing liver and kidney,and tonifying spleen and stomach as the main treatment principles and methods.The difference is that in ancient times,there were more descriptions of external evil invasion and other diseases causing osteoporosis,while in modern times,more attention is paid to the interaction of the liver,spleen,and kidney as diseases.3.Data mining of commonly used prescriptions in ancient and modern times shows that the treatment of osteoporosis mainly focuses on tonifying the liver,spleen,and kidneys.Ancient formulas mainly focused on tonifying,with wind dispelling,cold dispelling,and dampness removing as auxiliary methods.Modern formulas often use tonifying agents in combination with blood activating and stasis resolving drugs.
