The concept of disease disclosure belongs to the category of self disclosure.In health communication research,it often refers to patients’ self disclosure to others based on their own disease situation.Appropriate disease disclosure can help patients gain beneficial support and successfully adapt to the disease.At present,the incidence rate of chronic diseases in China is rising at a younger age,and the incidence rate of young people has increased in recent years.Against the background of China’s vigorous advocacy and implementation of chronic disease prevention and treatment,this study focuses on the practice of disease disclosure among young chronic disease patients,focusing on how disease disclosure occurs and develops among friends.By introducing Communication Privacy Management Theory,this study takes an interactive and process-oriented perspective to figure out the relationship between disease disclosure and privacy management by the specific situation.The in-depth interview method is the main research method of this study.In the pre-investigation phase,this study investigated 564 young patients with chronic diseases and their friends,and sought to understand the basic information about their cognition,attitude,and experience with chronic diseases.In the formal interview stage,a semi structured interview was conducted with 23 groups(A group of respondents includes a young patient with chronic diseases and a healthy friend who knows their condition)of interviewees(a total of 46 people).After coding and analyzing interview data,it was found that the key elements of the privacy management system drive the emergence and development of disease disclosure in young patients with chronic diseases.Before disease disclosure,the needs and motivations for disease disclosure are influenced by privacy management standards,helping patients form plans to establish a private and shared privacy boundary between them and their friends;In disease disclosure,patients and their friends apply autonomous or collaborative privacy control rules and strategies to shape both parties’ expectations for disease disclosure and privacy management based on managing privacy boundaries;After the disclosure of the disease,patients and their friends inevitably face privacy turbulence caused by ineffective or ineffective privacy controls,and adjust their experience,cognition,and coping behaviors with expectation gaps through differentiated results attribution.This process develops circularly in the process of balancing self concealment and disclosure,coordinating restraint and openness in friend relationships.During the process of disease disclosure,patients and their friends are not only the transmitter of disease information,but also the interpreter,manager,and creator of the meaning of disease and health.Based on the research findings,this study provides reference suggestions for co constructing a good environment for disease exposure.First,on a individual level,it is also necessary to enhance correct understanding and positive action on disease,self,interpersonal communication,and privacy management.Second,on the social level,it is also necessary to integrate multiple forces from improving the communication literacy of medical and related service personnel,the rigor and humanistic care of disease science popularization,and the service level related to the social security system.
