Autologous PRF Combined With Micro-skin In Promoting Wound Healing At Skin Donor Site After Skin Graft Surgery

Objective:Through comparing the treatment of autologous PRF combined with microparticle skin grafting with that of simple microparticle skin grafting in the donor area of the cases of chronic wounds treated with medium thickness skin grafting in our hospital,the advantages and disadvantages of PRF combined with autologous microparticle skin grafting are summarized,which provides a basis for the selection of wound repair schemes in the donor area of such patients.Materials and Methods:During the period from September 2020 to October 2022,60 patients with refractory wounds who met the screening criteria and underwent medium thickness skin grafting in our hospital were selected as the study subjects.They were divided into a control group and a experimental group according to a random number table method,with 30 patients in each group.After admission,treatment such as controlling blood sugar and blood pressure,controlling infection,and improving the overall condition was given.After completing relevant preoperative preparations,patients in the experimental group were treated with medium thickness skin grafts and grafting.The donor area of patients in the experimental group was treated with autologous PRF combined with microparticle skin grafting.The control group was treated with simple autologous microparticle skin grafting,followed by sequential coverage with vaseline gauze and sterile gauze,followed by pressure bandages,and routine dressing changes were started on the fifth day after surgery.The pain scores of the experimental group and the control group on the 5th,8th,11th,14th,and 17th days after surgery were compared using visual simulation scoring method;Compare the wound healing time of the donor site;Compare the hospitalization expenses of the two groups of patients during treatment;The incidence of postoperative adverse events was compared between the two groups;The patients were reexamined half a year after the surgery to compare the wound scars using the Vancouver Scar Assessment Scale method.Results:(1)General data of the two groups of patients:There was no statistically significant difference between the two groups in general data such as gender,age,BMI,blood sugar,white blood cell count,platelet count,hemoglobin value,albumin value,C-reactive protein value(P>0.05).(2)Pain severity score on the 5th,8th,11th,14th,and 17th day after operation:The visual analog pain score[(3.27±1.56)、(2.35±0.98)、(1.63±0.88)、(1.05±0.58)、(0.53±0.81)]of patients in the experimental group was lower than that in the control group[(4.08±1.83)、(3.24±1.09)、(2.73±0.94)、(1.85±0.98)、(0.93±0.48)],with a statistically significant difference(P<0.05)(3)Wound healing time of skin donor area:The wound healing time of the experimental group was(13.28±1.73)days,which was lower than that of the control group(16.03 ± 2.11)days,with a statistically significant difference(P<0.05)(4)Hospitalization cost:(27738.72±5298.27)yuan in the experimental group and(30176.28±6498.28)yuan in the control group B,with no significant difference between the two groups(P>0.05)(5)Occurrence of adverse events after operation:Follow up was carried out for 3-6 months after the operation of the two groups of patients to complete wound healing.The results showed that there were no adverse events(ulceration,pain or infection,or other discomfort symptoms)in the experimental group except for 3 patients who were not followed up,and no adverse events occurred in the control group except for 2 patients who were not followed up,with no statistically significant difference(P>0.05);(6)Scar in the donor site:The score of Vancouver Scar Assessment Scale in the experimental group(2.17±1.36)was lower than that in the control group(3.73±1.92),with a statistically significant difference(P<0.05).Conclusion:In this study,autologous PRF combined with microparticle skin was used to cover the donor site to promote healing.The results show that this method can reduce the pain of patients,accelerate the healing speed,and reduce scar hyperplasia without adverse events,and has good safety.It is a beneficial improvement on the existing wound treatment methods in the donor site,and it is worthy of clinical promotion and application.
