Objective:To summarize the clinical cases of wound closure with skin-stretching devices and provide clinical experience for wound closure.Methods:Twenty-two patients with skin and soft tissue defect were selected.Skin and soft tissue defect of head and limbs was caused by trauma,complications and chronic wound.The patients were randomly divided into the experimental group and the control group.There were 14 cases in the experimental group and 8 cases in the control group.The wound area of the experimental group was 4.06 cm2~11.75 cm2 with a width of 0.9 cm~4 cm,and the wound area of the control group was 5.17 cm 2~12.03 cm 2 with a width of 0.9~4.5 cm.The experimental group was treated with skin-stretching devices,while the control group was treated with free skin graft or local skin flap transfer after dressing change.The time of wound healing and the cost of treatment were monitored.Data were processed by statistics.Results:In the experimental group,the wound area was significantly reduced every week.Nine patients closed the wound completely after 3 weeks,and five patients closed the wound by silk suture after pulling the wound edge skin to a low tension state.The wound healing time was 23.21±5.08 days.Four patients in the control group healed after dressing change for 3 to 4 weeks,two patients healed after dressing change for 3 to 4 weeks,and two patients with bone exposure underwent local skin flap transfer for 5 to 7 days.The wound healing time was 25.62±5.75 days.There was no significant difference in wound healing time between the two groups(P>0.05).The total cost of treatment in the experimental group was 4754.07±625.19 yuan,while that in the control group was 7384.25±834.24 yuan.There was a significant difference between the two groups(P<0.05).Conclusion:Skin-stretching devices has a definite effect on the healing of skin defect wound,which can make the wound of small area of skin and soft tissue defect closed by direct traction and avoid surgical treatment;or the tension of the wound edge can be reduced after traction to suture directly and simplify the surgical method.Objective:To verify the effect of skin tension-reducing closure device on relieving hypochondria scar after surgical incision,and to provide a new strategy for clinical prevention and treatment of hypochondria scar.Methods:Thirty-seven cases of costal cartilage resection(all from secondary microtia surgery)were randomly divided into experimental group(group A)and control group(group B).The experimental group was treated with skin-stretching devices for three months after operation.The control group was not used skin-stretching devices after routine suture.Vancouver Scar Scale was used to evaluate the scars in the surgical area of the patients one month,three months and six months after operation.Data were processed by statisticsResults:19 cases in the experimental group and 18 cases in the control group were followed up one month after operation.One month after operation,the Vancouver Scar Scale score of 19 cases in the experimental group was 2.16~0.14,and that of 18 cases in the control group was 3.17±0.12,with significant difference(P<0.05).At 3 months after operation,5 patients in the experimental group were not visited due to insufficient length of distractor or delayed treatment,while 1 patient in the control group was not visited in time.The Vancouver Scar Scale score of 14 cases in the experimental group was 3.2±10.11 and that of 17 cases in the control group was 7.17±0.19,with significant difference(P<0.05).Six months after the operation,1 patient in the experimental group was lost due to untimely visit,and 2 patients in the control group were lost due to untimely visit.Six months after operation,the Vancouver Scar Scale score of 11 cases in the experimental group was 3.63±0.20,and that of 15 cases in the control group was 7.53±0.19,with significant difference(P<0.05).Conclusion:For patients with postoperative scar,skin-stretching devices can effectively reduce skin tension in the surgical area,and to some extent reduce the growth of hypochondria scar.
