Objective(s):The purpose of this study was to observe the clinical effect of alginate dressing combined with SSD dressing on wound healing in wound donor area,and to compare it with the traditional treatment in wound donor area in preventing wound infection,promoting wound healing,reducing wound pain during dressing change,improving the quality of wound scar and so on,as well as the corresponding economic cost.To explore the economical and effective healing methods for donor wound,and to provide reference for the treatment of donor skin wound in clinical trauma patients.Methods:In this study,60 patients who were to undergo skin grafting in the Department of Bone and Trauma in a Grade A hospital in Yunnan Province were randomly divided into control group(n=30)and intervention group(n=30)according to random number table.After the skin was removed by the surgeon in the operating room,the patients in the control group were pressed with aseptic gauze to stop bleeding,and then the wound was completely covered with Vaseline gauze,with three layers of cotton pad and bandage under pressure.After treatment with baking lamp for 40 minutes every day,the outer dressing was changed according to the amount of blood oozing on the cotton pad,and the patients were asked how they felt.If there was no abnormality,the dressing was opened on the 7th day and 14th day respectively to observe the wound healing.Until Vaseline gauze can be separated from the wound without injury.After the skin was removed by the surgeon in the operating room,the patients in the intervention group were pressed with aseptic gauze to stop the bleeding,then the wound was completely covered with SSD dressing as the primary dressing,and the alginate dressing was placed above the SSD dressing as the secondary dressing,the size was as small as possible not to exceed the edge of the wound,plus three layers of cotton pad and bandage.The amount of blood osmosis was observed every day after operation,and the outer dressing was changed according to the exudation area on the cotton pad.If the patient’s perception of the wound was normal,the inner dressing was changed on the 7th and 14th day after operation,and the wound healing was observed.The data of objective index(blood osmosis,wound healing time,infection)and subjective index(pain score and scar score three months after healing)were collected respectively in the two groups.Cost-effectiveness analysis was used to evaluate the two methods of wound treatment in the donor area.SPSS20.0 statistical software was used for statistical analysis,and the measurement data were expressed in x±s.The t-test,chi-square rank sum test and other methods were used for comparison.Statistical significance in P<0.05,and P<0.01 is significantly significant difference.Results:1.There was no significant difference in age,sex,skin area,skin site,serum albumin value and other general data between the two groups(P>0.05).2.The amount of bleeding 24 hours after operation in the intervention group was less than that in the control group,and the difference was statistically significant(P<0.05);In the control group,the dressing adhered closely to the wound surface on day 7 and 14,which could be easily removed in the intervention group.3.The median pain score for the first three days after surgery was 2 in the intervention group and 3 in the control group.,and the difference was statistically significant(P<0.01).4.The average healing time of the control group was(26.00±7.13)days,and that of the intervention group was(15.13±2.70)days,with statistically significant difference between the two groups(P<0.01).5.Three months after healing,the scar score of the intervention group was(27.63±3.69),the overall scar evaluation was(7.11±1.76),and the scar score of the control group was(34.18±7.36),the overall scar evaluation was(5.70±1.39),the difference between the two groups was statistically significant(P<0.01).6.The intervention component cost-effectiveness ratio was 280.67,and the control group’s cost-effectiveness ratio was 543.02.The intervention component cost-effectiveness ratio was lower than the control group,indicating that the intervention group had to spend less than the control group to achieve the same therapeutic effect.Conclusion(s):1.The combination of alginate dressing and SSD dressing can significantly shorten the wound healing time,reduce the pain,reduce the infection rate and improve the quality of wound scar compared with the traditional treatment.2.The combination of alginate dressings and SSD dressings can significantly improve the treatment effect of donor wounds,and at the same time,its economic cost is lower.Compared with traditional treatment methods,the total cost of hospitalization can be significantly reduced. |
