Part Ⅰ Clinical observation of Kang’ai injection in the treatment of colorectal cancerObjective:The clinical efficacy of Kang’ai Injection against colorectal cancer is explored by applying the thinking of TCM evidence-based treatment,combined with the means of clinical tests,by integrating and analyzing relevant data and observing the changes in patients’ clinical symptoms.It provides further basis for the selection of reasonable adjuvant therapeutic drugs for clinical anti-cancer.Methods:Sixty patients with advanced colorectal cancer treated in Yangzhou Chinese Hospital from October 2020 to December 2022 were selected and divided into a control group and an observation group according to the randomized control principle,with 30 patients in each group.The patients in the control group were given conventional chemotherapy plus targeted therapy,while the patients in the observation group were treated with Kang’ai injection on top of this.After 4 consecutive cycles of intervention,the tumour marker levels,Karlsberg scores,TCM evidence scores,quality of life scores and toxic side effects of the two groups were compared.Results:After 4 cycles of treatment,the levels of tumor markers CA19-9 and CEA in both groups were lower compared with those before treatment(P<0.05),and the levels in the observation group were lower than those in the control group after treatment(P<0.05);in terms of the Cascade score,both groups were significantly higher compared with those before treatment(P<0.05),and the score in the observation group was significantly lower than that in the control group after treatment,but the difference was not statistically significant(P>0.05),In terms of TCM symptoms,the observation group was better than the control group in improving palpitations and weakness(P<0.05),but no significant difference was found between the two groups in improving constipation,loose stools,physical wasting and poor appetite(P>0.05).In terms of side effects,the incidence of bone marrow suppression and gastrointestinal reactions in the observation group was significantly lower than that in the control group(P<0.05).however,there were no significant differences in the incidence of cardiotoxicity,peripheral toxicity,allergic reactions and alopecia between the two groups(P>0.05).Conclusion:When Kang’ai injection is combined with chemotherapy and targeted therapy for advanced colorectal cancer,it can make the clinical efficacy of patients improve,significantly improve clinical symptoms such as palpitation and weakness,and improve the quality of life of patients,with less toxic side effects to the body and higher safety.Part Ⅱ Mechanism of Kang’ai Injection against Colorectal Cancer based on network pharmacology and molecular docking technologyObjective:To study the mechanism of Kang’ai injection against colorectal cancer based on pharmacology and molecular docking technology.Methods:TCMSP、Uniprot、GeneCards and OMIM database were used to retrieve the target of Kang’ai injection in the treatment of colorectal cancer,Using the Venny website to make Venny diagrams and obtain common targets between diseases and drugs,and by Cytoscape 3.7.1 to map the drug-component-target network,Using the String website to construct drug-disease PPI(protein interaction)network maps and after GO enrichment analysis and KEGG enrichment pathway analysis,Auto Duck Vina was used to predict the core components and the key targets by molecular docking in the end.Results:87 active components of Kang’ai injection and 220 potential targets,1417 potential targets of colorectal cancer were obtained.GO enrichment analysis results showed the top 20 molecular functions of targets.The KEGG pathway enrichment results showed that the PI3K/AKT pathway、Hepatitis B、the TNF pathway、the Relax in pathway.Molecular docking showed that the key components of drugs are well docked with disease proteins.Conclusion:Kang’ai injection plays a role in the treatment of colorectal cancer through multiple genes and pathways,providing an objective basis for the effective clinical treatment of cancer by proprietary Chinese medicine.
