| Objective:To compare and explore the clinical efficacy of modified PPH and PPH in the treatment of severe prolapsed mixed hemorrhoids.Methods:The treatment status of 165 patients with mixed hemorrhoids received by the anorectal subspecialty of General Surgery,Southern Hospital of Southern Medical University from January to December 2020 was tracked by means of preoperative information survey,intraoperative follow-up and postoperative follow-up.Different groups were tested and analyzed.The experimental group(n=82)was treated with modified PPH,and the control group(n=83)was treated with PPH.The clinical efficacy and perioperative situations at one month postoperatively,symptom improvement before and one month after surgery,recurrence rate at one year after surgery,incidence of complications(including short-term and long-term),and cost-effectiveness(AC/E)were analyzed.Result:Results:The test results analyzed the advantages and disadvantages of two different surgical methods from the total effective rate of clinical efficacy,intraoperative blood loss,operation time and other parameters.1.From the aspect of the total clinical effective rate,the experimental group was 98.78%,while that of the control group was 91.57%,with significant difference(P<0.01);from the aspect of intraoperative blood loss,the experimental group was(10.22±4.13)ml,less than(22.05±7.32)ml in the control group(P<0.01);analyzed from the operation time,the experimental group was(57.73±13.53)min,shorter than(66.14±12.66)min in the control group(P<0.01);The number of days was analyzed,the experimental group was(4.60±0.49)d,shorter than(5.57±0.50)days in the control group(P<0.01);2.From the analysis of postoperative blood in the stool,prolapse,difficulty in defecation,thrombosed external hemorrhoids,and pain symptoms,the scores of the experimental group and the control group were significantly different(all P>0.05);3.Among the recent complications,postoperative pain,anal bulge,and abdominal discomfort,the incidence rates of patients in the experimental group were 8.54%,4.88%,and 2.44%,which were significantly lower than 24.10%,16.87%,and 15.66%in the control group(all P<0.05);4.In the long-term complications,the incidences of anal bulge and unclean defecation symptoms,the experimental group were 3.66%and 2.44%,significantly lower than that in the control group(15.66%and 13.25%)(all P<0.05);5.The value of cost-effectiveness of the experimental group were 13078.95,significantly better than 14414.11in the control group(P<0.05).Conclusions:For the treatment of mixed hemorrhoids,the effect of surgical treatment of the modified PPH is better than that of PPH,and the modified PPH surgery due to less blood loss,shorter operation time,postoperative recovery fast,shorter hospitalization time,cost/benefit ratio is better,and complications is less.Though the incidence of postoperative recurrence rate is basically identical conditions,the modified PPH surgery is clearly a better choice.Therefore,it is suggested to popularize and apply modified PPH. |
