Study On The Relationship Between Fluid Intelligence And Chronic Heart Failure

Abstract/Summary:
Objective To investigate if there is a fluid intelligence change in patients with Chronic Heart Failure(CHF)or not.Methods A total of 112 patients in the Department of Cardiology of Ordos Central Hospital from October 2020 to October 2021 were selected.Among them,77 patients with CHF who met the inclusion criteria were selected as the experimental group.The other 35 patients without CHF were selected as the control group,while the other 35 patients without CHF were selected as the control.The general information and Raven`s Standard Progressive Matrices(RSPM)scores of the two groups were compared.The patients with CHF were divided into NYHA II,III and IV levels according to the different cardiac functions.The RSPM scores of patients with different cardiac function levels were compared.According to different left ventricular ejection fraction(LVEF),the patients were divided into heart failure with reduced ejection fraction(HFrEF),heart failure with preserved ejection fraction(HFpEF),and heart failure with mildly reduced ejection fraction(HFmr EF).The RSPM scores of patients with different LVEF were compared.The relationship between LVEF,NT-proBNP and RSPM in patients with CHF was analyzed,and the relationship between RSPM and LVEF,NT-proBNP and RSPM was analyzed.Results There was a statistically significant difference in the cardiac function grade of non-CHF patients and CHF patients with LAd Ved LVed LVEF NT-proBNP NYHA(P <0.05).There was no significant difference between the control group and the experimental group in gender,age,smoking,drinking or hypertension,LVPWd,TC / TG / LDL-C /HDL-C / Lp(α)(P >0.05).The scores and total scores of " perception discrimination " "analogy comparison " " reasoning ability " " series relationship " and " abstract reasoning " in CHF patients were decreased(P<0.05).There was a significant difference in LVed / LVEF /Nt-proBNP between CHF patients with different cardiac function grades and those with different cardiac function grades.It can be considered that the LVed of cardiac function grade IV and cardiac function grade II and III are different.The longest LVed of grade IV cardiac function.It can be considered that the NT-proBNP levels of cardiac function grade IV and cardiac function grade II and III are different.Cardiac function grade IV NT-proBNP highest.There was no significant difference in gender,age,drinking or hypertension,LVPWd,TC / TG / LDL-C / HDL-C / Lp(α)between CHF patients with different cardiac function grades.There was a significant difference in the RSPM score between the patients with different cardiac function levels.The scores of " perception discrimination " and " series relationship " in patients with grade IV of central function were significantly lower than those in patients with grade II of cardiac function(P < 0.05),and the scores of " analogous comparison " and " reasoning ability " in RSPM was significantly lower than those in patients with cardiac function grade II and III(P<0.05),and there was no significant difference in the " abstract reasoning " score of CHF patients with different cardiac function grades.(P >0.05).It can be considered that LVed in HFpEF patients is different from that in the other two groups.The LVed of patients with HFpEF was the shortest(P<0.05).It can be considered that NT-proBNP in HFrEF patients is different from that in HFpEF patients.The levels of NT-proBNP in patients with HFrEF were higher than those in patients with HFrEF(P <0.05).Compared with the other two groups of patients.The scores of "perception discrimination," "analogy comparison" and "reasoning ability" " series relationship " and the total score of RSPM in HFrEF patients were the lowest(P < 0.05).There was a positive correlation between RSPM and LVEF(r = 0.732,P < 0.001),and there was a negative correlation between RSPM and NT-proBNP(r=-0.335,P= 0.003).There was no significant difference in RSPM single score and total score between CHF patients with different education levels(P>0.05).The increase of LVEF will reduce the risk of damage of FI level in patients with CHF,with statistical significance(OR = 0.703,95% CI 0.570-0.867,P =0.001).There was a significant difference in the level of FI between the two groups(AUC was 0.628,P = 0.047).The cut-off value of LVEF was 44%(sensitivity 60%,specificity66%)。Conclusion The total score of RSPM and the scores of each item in patients with CHF are decreased.With the increase of cardiac function classification and the decrease of LVEF.The total score of RSPM and the single score of RSPM were significantly lower than those of the control group.And RSPM score was positively correlated with LVEF.Negative correlation with NT-proBNP.There was no significant difference in RSPM single score and total score between patients with different education levels of CHF.The increase of LVEF will reduce the risk of FI damage in patients with CHF.LVEF has a diagnostic significance for the damage of FI level The cut-off value of LVEF was 44%(sensitivity 60%,specificity 66%)...
