The Predictive Value Of IL-6 And IL-10 For Febrile Seizures

Abstract/Summary:
Objective:To identify serum IL-6 and IL-10 expression levels and predictive value in children with febrile seizures.Providing direction to clinical practitioners for early disease diagnosis and improve understanding of the disease.Methods:From December 2020 to January 2022,34 children with febrile seizures(22 cases of simple febrile seizures and 12 cases of complex febrile seizures)in Yuncheng Central Hospital were selected as experimental groups,while selected 23 children with seizures without fever,34 children with fever without convulsion,and 22 healthy children as the control groups.Their clinical features and and expression of inflammatory cells in blood routine were collected,and venous blood was collected and centrifuged from children after a seizure within 24 hours and fever only or healthy children when they came here for diagnosis.The serum IL-6 and IL-10 levels of all study subjects were detected by flow cytometry,and SPSS 26.0 statistical software was used to analyze the obtained data.The levels of the two factors and the expression of inflammatory cells between the experimental groups and the control groups were compared.The non-normal distribution was expressed in median and 25 th,75th percentiles(M(p25,p75)),the Kruskcal Wallis test was used to compare the multi-group differences,and the Wilcoxon test was compared with the difference between the two groups.The conforming normally distributed count data was expressed as mean ± standard deviation(`X±SD),and the t-test is performed between the two groups.The correlation in different group between the two factors,inflammatory cells and the clinical features of the febrile convulsive group were analyzed by Spearman rank correlation analysis.The predictor value of serum IL-6,IL-10 and neutrophil to lymphocyte ratio for febrile seizures was analyzed by receiver operating characteristic curve.Results:Serum IL-6 level in the febrile convulsions group was increased compared with those in the seizure group and the healthy control group,and the difference was statistically significant(P<0.05).The serum IL-10 level in febrile seizures group was higher than that in the healthy control group,and the difference was statistically significant(P<0.05).There were no statistically difference between the two types of febrile seizures(P > 0.05).Children with febrile seizures had a statistically significant increase in neutrophil ratio and neutrophil to lymphocyte ratio and a statistically significant decrease in lymphocyte ratio compared to the three control groups(P<0.05).There was a positive correlation between IL-6 and IL-10 in serum of children with simple febrile seizures and convulsions without fever(rs=0.53,rs=0.73,P<0.05),while no statistically significant association between the two factors in febrile seizure and neutrophil ratio,lymphocyte ratio and neutrophil to lymphocyte ratio(P>0.05).There was no statistical association between the clinical features and study factors in the febrile convulsive group(P>0.05).The optimal cut-off value for the prediction of febrile seizures in serum IL-6 was 17.519 pg/ml(sensitivity 73.5%,specificity 67.1%).The optimal cut-off value to predict febrile seizures in serum IL-10 was 6.077 pg/ml(sensitivity 67.6%,specificity 64.6%).The optimal cut-off value for predicting febrile seizures with neutrophil to lymphocyte ratio was 1.6(sensitivity 79.4%,specificity60.8%).The overall predictive value of the three combined for febrile convulsions was0.789,which was higher than the prediction of each indicator alone.Conclusion:Serum IL-6 and IL-10 levels in febrile seizures were abnormally expressed,and the expression levels of these were increased.They had a positive correlation in simple febrile seizures.Increased likelihood of febrile seizures is suggested when serum IL-6>17.519 pg/ml,IL-10>6.077 pg/ml,and neutrophil to lymphocyte ratio > 1.6.The combined detection of the three can improve the sensitivity and specificity of the prediction of febrile convulsions.All of them are valuable auxiliary diagnostic indexes of febrile convulsions.Changes in these indicators should be noted by clinicians when diagnosing the disease.
