Correlation Analysis Of Serum 25(OH)D,IL-6 And Related Serological Indicators With Febrile Seizures

Abstract/Summary:
Objective:To study the expression levels of serum 25-hydroxyvitamin D[25(OH)D],Interleukin-6(IL-6)and related serological indicators in children with febrile seizures,and to explore their roles and mechanisms in febrile seizures.To provide a basis for clinical prevention and control of the occurrence and development of febrile seizures.Methods:From October 2020 to June 2022,60 children with Febrile seizure hospitalized in the Department of Pediatrics of our hospital were selected as the subjects of the febrile seizure(FS)group,and they were divided into simple febrile seizures (SFS)group and complex febrile seizures(CFS)group.There were 43 cases of SFS and17 cases of CFS.During the same period,75 children with fever without convulsion diagnosed as respiratory tract infection were selected as the control group.The relevant clinical data(age,sex,cause of disease,course of disease,serum 25(OH)D,IL-6 level and other serological index values,etc.)of children in FS group and control group were collected.The correlation of serum 25(OH)D,IL-6 and other serological indicators between FS and the control group and between SFS and CFS groups was compared.The factors related to the occurrence of febrile seizures were determined by binary Logistic regression analysis,and the predictive value of these factors for the occurrence of febrile seizures was analyzed by ROC curve.Results:1.There was no significant difference in gender,age and course of disease between FS group and control group(P>0.05).2.The duration of seizure in CFS group was longer than that in SFS group,and the difference was statistically significant(P<0.05).There were no significant differences in gender,age,duration of fever before seizure,body temperature at seizure onset,and FS family history between the SFS group and the CFS group(P>0.05).3.The serum 25(OH)D level of children with febrile seizures was different in different age groups,the 0~1 year old group had the highest 25(OH)D level,and the 5~6 year old group had the lowest 25(OH)D level,the difference between the two groups was statistically significant(P<0.05),and there was no significant difference between the other age groups(P>0.05).4.The serum 25(OH)D level in CFS group was 26.80(22.86,39.27)ng/ml,which was significantly lower than 43.51(31.70,55.49)ng/ml in SFS group(P<0.05).IL-6, Neutrophils/lymphocytes ratio(NLR),Platelet/lymphocyte ratio(PLR),Kalium(K+),Sodium(Na+),Chlorine(Cl-)and Calcium(Ca2+)had no significant difference between SFS group and CFS group(P>0.05).5.The serum IL-6,NLR and PLR in FS group were significantly higher than those in control group,The levels of serum 25(OH)D,Na+and Cl-in FS group were significantly lower than those in control group(P<0.05).There was no significant difference in K+and Ca2+between FS group and control group(P>0.05).6.Binary Logistic regression analysis showed that 25(OH)D(OR=0.963,P=0.016)and Na+(OR=0.700,P=0.022)were protective factors for FS.7.The ROC curve showed that the best cut-off value of 25(OH)D was 51.59,the sensitivity was 58.7%,the specificity was 78.3%,95%CI(0.630-0.803)%,AUC was 0.717(P<0.001).The best cut-off value of Na+was 138.45,the sensitivity was 65.3%,the specificity was88.3%,95%CI(0.736-0.883)%,AUC was 0.809(P<0.001).Na+had the largest area under the curve and was the best predictor of FS occurrence.Conclusions:1.Serum 25(OH)D and Na+are closely related to the occurrence of febrile seizures.25(OH)D and Na+are protective factors for febrile seizures,and the decrease of Na+is a predictor of febrile seizures.2.The deficiency of serum 25(OH)D level is particularly obvious in children with complex febrile seizures,which should be supplemented.3.Serum IL-6 level is related to the occurrence of febrile seizures,and is an important inflammatory cytokine in the occurrence of febrile seizures,but has no significant role in distinguishing the type of seizures.
