Correlation Analysis Of Uric Acid、homocysteine And Serum Cystatin C Level With BaPWV And CIMT In People With Normal High Blood Pressure And Type 2 Diabetes Mellitus

Abstract/Summary:
Objective:1.To analyze the general clinical characteristics and metabolic indexes such as age,body mass index,blood sugar,glycosylated hemoglobin,diabetes duration,blood lipids,Cystatin C,uric acid,homocysteine,brachial-ankle pulse wave velocity,Carotid intimamedia thickness in patients with normal high blood pressure combined with T2DM,patients with normal blood pressure combined with T2DM,and patients with hypertension complicated with T2DM.2.To investigate the correlation between brachial-ankle pulse wave velocity and carotid intima-media thickness and uric acid,homocysteine and serum Cystatin C level in patients with normal high blood pressure complicated with T2DM,and to study the influencing factors.Methods:According to the selection and exclusion criteria,A total of 138 patients with normal high blood pressure complicated with T2DM who visited the outpatient department,inpatient department and physical examination center of the First Hospital of Shanxi Medical University from June 2020 to August 2021 were selected.Meanwhile,126 patients with normal blood pressure complicated with T2DM and 150 patients with hypertension complicated with T2DM were selected during the same period.The general clinical data and serological indexes such as triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C)were collected Low density lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG),2-hour postprandial blood glucose(PBG),glycosylated hemoglobin(HbA1c),uric acid(UA),homocysteine(Hcy),Cystatin C(Cys C),brachial-ankle pulse wave velocity(baPWV),carotid intima-media thickness(CIMT)and other data were collected,Statistical analysis was performed to compare the differences of general clinical data and serological indexes,baPWV and CIMT among the three groups,Correlation analysis and multiple linear regression analysis were used to explore the correlation between baPWV,CIMT and uric acid,homocysteine and serum Cystatin C level.Results:1.The age of high normal blood pressure combined with T2DM group and hypertension combined with T2DM group was significantly older than that of normal blood pressure combined with T2DM group(P<0.05).BMI increased in turn among the three groups(P<0.05).The duration of diabetes in patients with hypertension combined with T2DM was significantly longer than that in normal blood pressure combined with T2DM group and normal high blood pressure combined with T2DM group(P<0.05).2.There was no significant difference in HDL-C,FBG and HbA1 c among the three groups(P>0.05);The values of TC,TG,LDL-C and Cys C in high normal blood pressure combined with T2DM group and hypertension combined with T2DM group were significantly higher than those in normal blood pressure combined with T2DM group(P<0.05).And the PBG value of hypertension combined with T2DM group was significantly higher than that of normal blood pressure combined with T2DM group and normal high blood pressure combined with T2DM group(P<0.05).The UA and Hcy values of normal blood pressure combined with T2DM group,normal high blood pressure combined with T2DM group and hypertension combined with T2DM group increased successively(P<0.05).3.The comparison results of baPWV and CIMT showed that baPWV in normal high blood pressure combined with T2DM group and hypertension combined with T2DM group were significantly higher than that in normal high blood pressure combined with T2DM group.There was no significant difference between normal high blood pressure combined with T2DM group and hypertension combined with T2DM group(P>0.05).There was significant difference in CIMT value among normal blood pressure combined with T2DM group,normal high blood pressure combined with T2DM group and hypertension combined with T2DM group,and increased in turn(P<0.05).4.The baPWV value of normal high blood pressure combined with T2DM group was correlated with age,BMI,duration of diabetes,PBG,SBP,Cys C and HCY.The CIMT value was correlated with age,BMI,duration of diabetes,PBG,BMI,SBP,DBP,LDLC,Cys C,HCY and UA.5.Multiple linear regression analysis showed that age,PBG,SBP and Cys C were the influencing factors of baPWV in normal high blood pressure combined with T2DM group,and age,SBP,PBG,Hcy and Cys C were the influencing factors of CIMT in normal high blood pressure combined with T2DM group.Conclusion:1.People with high normal blood pressure and hypertension combined with T2DM have a higher risk of macrovascular damage than those with normal blood pressure combined with T2DM,so intervention should be carried out early,which is very beneficial for controlling macroangiopathy in this part of the population.2.Arteriosclerosis is associated with age,BMI,duration of diabetes,PBG,SBP,Cys C and HCY in patients with normal high blood pressure combined with T2DM.The arterial stiffness can be assessed by evaluating age,BMI,duration of diabetes,PBG,SBP,Cys C and value.Carotid atherosclerosis correlated with age,BMI,duration of diabetes,PBG,SBP,DBP,LDL-C,Cys C,HCY and UA.Carotid atherosclerosis can be evaluated according to age,BMI,duration of diabetes,PBG,SBP,DBP,LDL-C,Cys C,HCY and UA values.3.Age,PBG,SBP and Cys C are the influencing factors of baPWV value in normal high blood pressure combined with T2DM group,and age,SBP,PBG,Hcy and Cys C are the influencing factors of CIMT value in normal high blood pressure combined with T2DM group.
