Study On Rules Of Syndrome Differentiation And Prescription Of Famous Veteran TCM Practitioner Hu Ke In Treating Distention And Fullness Based On Date Mining Technology

Objective:Using the TCMISS(Traditional Chinese Medicine Inheritance Support Platform)(V2.50),a systematic study was conducted on the cases of distention and fullness treated by Professor Hu Ke,a famous veteran TCM practitioner in Jiangxi Province,to summarise his treatment and prescription rules,with a view to better inheriting and carrying forward Professor Hu Ke’s experience in the treatment and related academic ideas of distention and fullness,and to provide some reference for future treatment of TCM and research on distention and fullness.Methods:A retrospective analysis is conducted on the cases of distention and fullness of the abscess treated by Professor Hu Ke between September 2019 and October 2021.Using the TCMISS(V2.50),the collected and collated case data were entered into this system to establish a database of distention and fullness containing patient ’ s basic information,information on the four TCM consultations,and herbal prescriptions,etc.Through the internal algorithm of the platform,make statistics on the database information,and then according to the statistical results,combined with the Professor Hu Ke’s clinical experience for in-depth analysis,and dig out the relevant medication rules of Professor Hu Ke in the treatment of distention and fullness.Results:(1)A total of 301 cases were included in this study.Through the statistical results of the platform,it was found that the number of female patients was more than that of male patients,and female patients accounted for 58% of the total number;The onset age of distention and fullness is concentrated between 30 and 69 years old,and the age group with the largest number is 40 to 49 years old,accounting for 20.25% of the total.(2)The symptoms that appeared more than 50 times in patients with distention and fullness included gastric distension,belching,dry mouth,gastric distension aggravated after eating,irregular bowel movements,bitterness in the mouth,acid reflux,burning sensation,slightly poor sleep,poor sleep,easy fatigue,vexation,and average food intake.(3)The tongue of the patient with distention and fullness is mainly light red with thin white coating,and the pulse is mainly string,sunken,slippery,and number of strings.(4)The most common types of symptoms of distention and fullness were liver and stomach disharmony,spleen and stomach weakness,spleen deficiency and dampness,and cold and heat disorder,with liver and stomach disharmony occurring most frequently.(5)All the prescriptions entered in this study involved a total of169 herbal medicines,and 20 herbal medicines were used more frequently than 50 times,and those used more frequently included Bupleurum,Rhizoma Pinellia Praeparatum,Codonopsis pilosula,Radix Scutellariae,Radix Paeoniae Alba,fried fructus aurantii immaturus,betel nut,Rhizoma Cyperi,Radix Aucklandiae,fried malt,fried grain bud,tangerine peel,Gallus gallus domesticus and Magnolia officinalis.According to the classification of herbal medicines,nature,taste and meridian,it is found that in terms of drugs classification,Qi regulating drugs and deficiency tonifying drugs are mostly used;in terms of four qi,most of the drugs were warm and cold;in terms of five flavors,most of the drugs were pungent and bitter;in terms of meridians,most of the drugs entered the spleen,stomach,liver,lung and large intestine meridians.(6)Using the built-in rules of the platform data analysis system,after setting the reliability to 0.9 and the support to 240,it is concluded that the commonly used drug combinations of Professor Hu Ke are Radix Bupleurum,Rhizoma Pinellia Praeparatum,Codonopsis pilosula,Radix Scutellariae,Radix Paeoniae Alba,parched Aurantii Immaturus,betel nut,Rhizoma Cyperi.(7)Using the platform data analysis system,based on the analysis of drug association rules,the correlation degree is set to 9,and the punishment degree is set to 3.By using the complex system entropy clustering and improved mutual information method,13 new drug combinations of Professor Hu Ke for treating fullness are extracted,which are: Sequence No.1: Radix Paeoniae Alba,Fructus Amomi,Pericarpium Citri Reticulatae Viride,Bupleurum chinense,parched Fructus Aurantii Immaturus;Sequence No.2:Pericarpium Citri Tangerinae,Fructus Forsythiae,Fructus Gardeniae,Cortex Magnolia Officinalis;Sequence No.3: Ophiopogon japonicus,Pinellia ternata,white lentils,lily bulb;Sequence No.4:Radix Codonopsis,Radix Pseudostellariae,Rhizoma Cyperi,Radix Glehniae;Sequence No.5:Jujube,Rhizoma Atractylodis,Ramulus Cinnamomi,Zingiberis Rhizoma,Coptidis Rhizoma;Sequence No.6:Jujube,ginger,cassia twig and picrorhiza picrorhiza;Sequence No.7:Fructus Amomi,Pericarpium Citri Reticulatae Viride,Atractylodes macrocephala,Scutellariae Radix,Bupleuri Radix;Sequence No.8:Coix seed,polygala tenuifolia,Polygonum multiflorum Thunb,Ganoderma lucidum;Sequence No.9:Coix seed,Acorus tatarinowii,Polygonum multiflorum,Ganoderma lucidum;Sequence No.10:Radix Aconiti Lateralis Preparata,Bupleuri Radix,Radix Glycyrrhizae Preparata,Scutellariae Radix,parched Fructus Aurantii Immaturus;Sequence No.11:Radix Aucklandiae,Rhizoma Dioscoreae,Radix Pseudostellariae,Rhizoma Cyperi,parched malt,Radix Aconiti Lateralis Preparata,and Areca;Sequence No.12:Jujube,Zingiberis Rhizoma,Coptidis Rhizoma,Magnolia Officinalis,and Fructus Gardeniae;Sequence No.13:Rhizoma Pinelliae Preparata,Rhizoma Dioscoreae,Herba Dendrobii,Rhizoma Polygonati and Radix Glehniae.Conclusion:Professor Hu Ke believes that distention and fullness is mostly in the qi division,although there are differences between qi stagnation and qi deficiency,but the pathogenesis is mostly a malfunction of qi elevation in the middle jiao,so the treatment is based on draining the liver、regulating qi and harmonizing the stomach,reflecting the treatment concept of "treating the stomach and not forgetting soothing the liver".By examining the evidence and investigating the causes and using flexible medication,the drugs mostly have the functions of regulating qi,harmonizing the stomach,tonifying Qi,clearing heat and drying and dampness.At the same time,Professor Hu Ke pays attention to preaching and educating the patients,regulating emotions,caring for the gastrointestinal tract,and improving or even eliminating the symptoms of the patients.Professor Hu Ke’s clinical diagnosis and treatment focus on syndrome-based investigation and syndrome differentiation,and the prescription is rigorous and flexible,which are worth learning.
