| Objective:The effect of Chaihu-Shugan-San on shoulder joint pain and range of motion after rotator cuff repair surgery was observed,and a new idea was provided for pain after rotator cuff repair from Shugan.Methods:From June 2021 to January 2022,60 patients who underwent arthroscopic rot ator cuff repair in the Department of Sports Medicine of the Second People’s Hospital of Fujian Province were included,and 30 patients were randomly divided into an ex perimental group and a control group.The control group was given basic treatment(i ntravenous instillation flubiprofen+rehabilitation exercise)after surgery,and the exper imental group was given oral administration of basic treatment combined with ChaihuShugan-San(granules).the assessment of symptomatic changes was assessed using Eval uation scale for Qi stagnation and blood stasis syndrome based on patient reported out comes and the TCM Syndrome Integral Scale,and the Pain Assessment was assessed using VAS(Visual Analogue Score)scoring system.Shoulder range of motion assessm ent includes anterior flexion,abduction,internal rotation,and external rotation range of motion.Result:1.There was no statistical difference in gender,age,BMI,limb laterality,diagnosis score of stagnant blood stasis,and preoperative VAS score of the two groups(P>0.05).2.In this study,the scores shown in the PRO scale of stagnant blood stasis in the experimental group decreased from 40.53±6.54 before surgery to 20.23±3.88 from 4 weeks after surgery,and the symptomatic score decreased from 10.60 ± 1.79 before surgery to 6.10±1.06 in the 4th week after surgery,and there were significant differences in the degree of stasis and the degree of TCM-related symptoms in the experimental group and the control group at 1 week,2 weeks and 4 weeks after surgery(P<0.05),Chaihu-Shugan-San can effectively relieve liver depression,relieve pain,activate blood stasis,and have a certain clinical effect on qi stagnation and blood stasis and related symptoms.3.There was no statistical difference in VAS scores between the two groups on day 1 after surgery(P>0.05).The VAS score on the first day after surgery was 5.27±0.64 points,3.43 points ± 0.57 points in 1 week after surgery,2.50 points ± 0.63 points in 2 weeks after surgery,1.87 ±0.63 points in 4 weeks after surgery,5.30 ±0.60 points on the first day after surgery in the control group,4.17 ± 0.79 points in 1 week after surgery,3.30 ± 0.79 points in 2 weeks after surgery,and 2.60 ±0.93 points in 4 weeks after surgery There were statistical differences in VAS scores between the two groups at 1 week,2 weeks and 4 weeks after surgery(P<0.05).4.There was no statistical difference in passive mobility between the two groups on day 1 after surgery(P>0.05).The activity activity of passive anterior flexion,abduction,external rotation and internal rotation gradually improved during the postoperative follow-up,and the activity of the 1 week,2 week and 4 weeks after surgery was significantly better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Chaihu-Shugan-San significantly alleviates the degree of symptoms related to stagnant blood stasis in patients with rotator cuff repair,effectively reduces the pain after rotator cuff repair,and has a clear positive effect on improving the passive mobility of postoperative shoulder joint anterior flexion,abduction,external rotation,internal rotation and so on. |
