Clinical Observation Of Two Treatment Methods For Distal Femoral Fractures

Abstract/Summary:
Objective: The purpose of this question is to combine the retrograde intramedullary nail(RIN)internal fixation with the minimally invasive percutaneous plate osteosynthesis(MIPPO)technology combined with the locking compression plate(Locking compression plate),which conforms to the concept of "equal emphasis on muscles and bones" in traditional Chinese medicine.Plate,LCP)internal fixation in the treatment of distal femoral fractures,the clinical efficacy was compared,and the advantages and disadvantages of the two treatment methods were compared.To study and analyze the guiding role of TCM theory of "equal emphasis on muscles and bones" in the clinical treatment of distal femoral fractures,and to provide theoretical basis for the future treatment of distal femoral fractures with integrated traditional Chinese and Western medicine.Methods: A retrospective analysis was made of 57 patients with distal femoral fractures admitted to the Department of Orthopedics,Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2019 to April 2021,including 31 patients treated with RIN and 26 patients treated with MIPPO technology combined with LCP plate.There was no significant difference in preoperative general data and basic information between the two groups(P>0.05).Comprehensive clinical data comparisons were made on the length of surgical incision,operation time,intraoperative blood loss,postoperative pain,clinical healing time of fractures,and the score of the Hospital for Special Surgery(HSS)of knee joint function.Knee joint function HSS score was performed at outpatient re-examination at 1,3,6,and12 months after operation,and SPSS25.0 statistical software was used for statistical analysis.Results: The RIN group was superior to the MIPPO combined with LCP plate group in terms of incision length,operation time,intraoperative blood loss,and clinical fracture healing time(P<0.05),and the difference was statistically significant.There was no significant difference in clinical fracture healing time between the two groups(P>0.05).In terms of HSS score of knee joint function between the two groups,at 1 month after surgery,the RIN group was better than the MIPPO combined with LCP plate group(P<0.05),and the difference was statistically significant.Significant statistical difference;12 months after operation,MIPPO combined with LCP plate group was better than RIN group(P<0.05),the difference was statistically significant.Conclusion: The RIN group has more advantages in terms of incision length,operation time,intraoperative blood loss,and clinical fracture healing time.RIN has better biological stability,and patients can perform functional exercise of the affected limb at an early stage.Therefore,the early postoperative RIN The group could obtain higher knee joint function HSS score.The MIPPO combined with LCP plate group could achieve better articular surface reduction,and the affected limb could achieve better function in the later period of recovery.There was no significant difference in postoperative pain between the two groups.
