Objective: In this paper, through the comparison of Expert Tibial Nail and Minimally invasive percutaneous osteosynthesis for treatment of distal tibial fractures, to investigate the curative effect of treatment of fractures of the distal tibia for expert tibial nail, provides the reference for the clinical treatment of distal tibial fractures.Methods: This paper selects the Department of orthopedics of Affiliated Hospital of Yan’an University from 2009 to 2013, 98 cases of distal tibial fracture were analyzed retrospectively. Including 51 cases of distal tibial fracture with closed reduction and internal fixation with ETN, 47 patients with small incision minimally invasive percutaneous plate screw fixation, by comparing the two kinds of surgical treatment of distal tibial fracture operation time, operation amount of bleeding, postoperative weight-bearing time, fracture healing time, complications and related after operation, postoperative ankle function score and tibial shaft fractures Johner-Wruhs evaluation results, statistical analysis of the above data, the result of P<0.05 with significant difference, to assess the value of ETN in treatment of distal tibial fractures postoperative effect.Results: In this study, a total of 98 cases of patients, all patients were followed up for 6-18 months. 98 cases of distal tibial fracture patients obtained bone healing. The operative time in the treatment of ETN fractures of the distal tibia was(93.24 + 22.53) min, blood loss was(154.40 + 40.30) ml, intraoperative fluoroscopy times(19.3 + 5.62); and the operative time of MIPPO for the treatment of distal tibial fractures were(90.16 + 17.90) min, blood loss was(168.42 + 38.50) ml; intraoperative fluoroscopy time was(18.2 + 4.86), there was no significant difference of these data(P>0.05). After ETN therapy in patients with limb full weight-bearing time was(8.50 + 1.50) months, the bone healing time was(19.2 + 3.2), using MIPPO for the treatment of distal tibial fractures with full weight-bearing time was(10.62 + 1.58) months, the bone healing time(24.8 + 8.9), which treated by ETN distal tibial fractures in patients with limb full weight-bearing time and bone healing time was less than patients treated in the MIPPO, there were significant differences between the two, or P<0.05. The ankle function score was used after operation and the fracture of shaft of Johner-Wruhs assessment of the results of analysis, no significant difference between ETN group and MIPPO group, P>0.05.Conclusion: ETN is for the treatment of distal tibial fracture is one of the ideal method, it has the characteristics of small firm fixation of the fracture, soft tissue injury, completely suitable for distal tibial skin and soft tissue condition is poor, the patients, it is placed at a distance of a plurality of multi direction of ankle in near distance, multi dimensional level locking nail, the fracture of the distal fracture stability, protecting the blood supply, reduce the fracture malunion, nonunion and delayed union of the incidence, clinical is a kind of ideal material of internal fixation.
